On June 6, 2014 the Division of Water Resources (DWR) received your information for the above referenced
project. The DWQ has determined that your application was incomplete and /or provided inaccurate
information as discussed below.
Additional Information Requested:
® A review of the General Certification GC3885 (NW13) reflects the need for a $570 permitting
fee based on stream impacts over 150 linear feet. For activities meeting any one (1) of the
thresholds or circumstances listed in the certification, written approval is required from the
DWR. Because your submittal includes impacts to a stream and a buffer in the Neuse Basin,
and activities proposed within the buffer are not considered "exempt ", according to item f) in
GC3885, written approval for a 401 Water Quality Certification is required and must include the
appropriate fee. Please remit a check for $570 to INC DWR, 401 & Buffer Permitting Unit, 1617
Mail Service Center, Raleigh, NC 27699 -1617.
Please also submit 4 copies of updated information on page 1 of 10 to reflect the types of
approval sought ( #1d) and that the application is for written approval (1e) so that we may
continue processing your application.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please
be aware that any impacts requested within your application are not authorized (at this time) by the DWR.
