forwarding herewith a modified Attachment B, certified May 23, 2013, to the Town of Angier for the
subject residuals land application program.
This certification is being issued to modify the Attachment B certification to combine:
• Fields 03a and 03b into one Field 03 with acreage of 8.1 and a maximum yearly
application rate of 52.0 inches.
• Fields 04a, 04b, and 04c into one Field 04 with acreage of 8.72 and a maximum yearly
application rate of 52.0 inches.
• Fields 05a, 05b, 05c, 05d, 05e and 05f into one Field 05 with acreage of 13.68 and a
maximum yearly application rate of 52.0 inches.
Please replace the existing Attachment B of Permit No. WQ0002638 that was previously
certified on April 4, 2008, with this modified Attachment B. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment B Permit No.
WQ0002638, issued on January 3, 2008, and making sure to retain any vicinity and buffer maps for any
existing and unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0002638 that was previously certified on April 4, 2008; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above-
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last

herewith Permit No. WQ0002638, dated January 3, 2008, to The Town of -Angier for the continued
operation of the subject wastewater treatment and surface irrigation facilities. •
This permit shall be effective from thel date of issuance until December 31, 2012, shall void
Permit No. WQ0002638 issued August 22, 2003•, and shall be subject to the conditions and limitations' as
specified therein. Please pay particular attention to the monitoring requirements in this permit: Failure to
establish an adequate system for collecting and maintaining the required operational information will
result in future compliance,problems. . . .
Note permit conditn I.1 requires The Town to: submit to. the Division updated site maps. ' The •
information must be submitted to both the Central and•Regional Offices, ' •
For your review a' copy of a site map and soils map. as prepared by the Fayetteville Regional .
Office is included with this letter. This' map indicates that Fields 3, 4,' and 5 have been divided into.
subfields. Interpolation of the site map •and soils map indicate that the subfields have varying soil types
and it may be appropriate to assign different loading: rates to the different fields. In addition, it appears
that the permitted acreage may have changed. This will need to be verified, and corrected if needed. • If
modifications to this permit are required to reflect the updated site maps, the Town must also submit an

application for a modification to the permit. '
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming 'to Chapter 150B of the

FOR THE
continued operation of a 258,000-GPD surface irrigation -treatment arid disposal facility consisting of 210 •
linear feet of 12-inch gravity sewer, a 450-GPM pump station with dual450=GPM pumps, approximately
620 linear feet of 8-inch force main, flow metering, an 8-acre facultative lagoon with a natural clay
having a hydraulic conductivity of 1 x 10-6 cm/sea or less, chlorination facilities, 'dual 700-GPM irrigation •
pumps with approximately 3,460 linear feet of 8-inch force main; a solid set spray :irrigationsystem which-.
includes 36,000 linear feet of piping on a 46 acre sprayfield, approximately 1,585:linear feet of 2.5-inch.
water line, approximately 970 linear feet of 2-inch water line, and all associated piping, ' valves, controls, '
and appurtenances :to serve the Town of Angier, with no discharge of wastes' to the surface waters, .
pursuant to the application received September 26, 2007,• and subsequent additional information received •
by the Division of Water Quality (Division), and .in conformity with the project plan, specifications, and.
other supporting data subsequently filed and approved bq the Department of Environment and Natural •
Resources and considered a part of this permit. • • . •
This permit shall be effective from the date of issuance until December 31, 2007, shall void Permit
No. WQ0002638 issued August 22, 2003, and shall be subject to the following specified conditions and •
limitations: •

be back-up ORC(s) of the facilities in accordance with 15A NCAC 08G .0200. The ORC shall visit •
the facilities in accordance with 15A NCAC 08G .0200 or as specified in.this permit and shall comply
with all other conditions specified in these rules. - • • '
3. A suitable year round vegetative cover shall be maintained such that crop .health is optimized, allows: + • :: •

4. Adequate measures shall be taken to prevent wastewater ponding or runoff from.the irrigation field.- ..
5. Irrigation shall not be performed during inclement weather or when the' ground is in a condition that 1;
will cause ponding or runoff. .:.:
6. All waste application equipment must be tested and calibrated at least once per -year. Records of the.
calibration must be maintained for five years. . .
7. No type of wastewater other than that from Town of Angier shall be in-igated onto the irrigation area. ••

5; The Permittee tracking the amount of wastewater disposed shall maintain adequate records. These
records shall include, but are not necessarily limited to, the following information: •
- a. Date of irrigation, ..t!.. „ i .
b. Volume of wastewater irrigated,
c. Field irrigated,
d. Length of time field is irrigated,
e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field,
f. Weather conditions,
g. Maintenance of cover crops,
h. Field number consistent with the soil type and application rate, and •

i. Wetted area acreage for each individual field consistent with the soil type and application rate.

6. Freeboard (waste level to the lowest elevation on the top of the embankment) in the facultative lagoon
shall be recorded weekly. •
7. Monitor wells shall be sampled after construction and within 3 months prior to initiating waste
disposal operations. Monitor wells shall be sampled thereafter at .the frequencies and for the
parameters specified in Attachment C. All mapping, well construction forms, well abandonment
forms, and monitoring data shall refer to the permit number arid the well nomenclature as provided in
Attachment C. •
8. For the initial sampling of the well as specified elsewhere. in the permit, the permittee shall submit a
copy of the GW-1 Form (Well Construction Record) that lists this permit number and the appropriate
monitoring well identification number with the Compliance Monitoring Form (GW-59) for that well.
Initial Compliance Monitoring Forms that do not iriclude'copies.of.the GW-1 form. are incomplete
and may be:returned to the permittee.without being processed:
9. ' Two (2) copies of the results of the sampling and analysis :must ,.be received on Forin GW-59
(Groundwater Quality Monitoring: Compliance Report Form); along with attached copies of
laboratory analyses, by the Division of Water Quality, Information Processing Unit, .1617 Mail
Service Center, Raleigh, North Carolina 27699-1617 on or before.the'last working day of the month
following the sampling month. Form GW-59 shall include the number of this permit and the
appropriate well identification number. One Form GW-59a certification form shall be provided for
each set of sampling results submitted. '
10. Three (3) copies of all monitoring data [as specified in Conditions IV. 3. and IV. 4.] on Form NDMR
• for each PPI and three (3) copies of all operation and disposal records [as specified in Conditions IV.
5 and IV. 6.] on Form NDAR-1 for every field shall be submitted on or before the last. day of the
following month. If no activities occurred during the monitoring.month, monitoring reports are. still
required documenting the absence of the activity. All information shall be submitted to the following

2. This permit shall become voidable unless the facilities are+ constructed .in accordance. withithe
conditions of this permit, the approved plans and specifications, and other supporting data. - I
3. This permit is effective only with respect to - the nature• and volume of wastes . described. in the
application and other supporting data. No variances 'to applicable rules governing the construction
• and / or operation of the permitted facilities are granted unless specifically requested and granted in
this permit.
4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other government agencies (local, state;
and federal) that have jurisdiction. Of particular concern to the Division are applicable river buffer
rules in 15A NCAC 02B .0200, erosion and sedimentation control requirements in 15A NCAC
Chapter 4 and under the Division's General Permit NCG010000, and any requirements pertaining to
wetlands under 15A NCAC 02B .0200 and 02H .0500.
5. In the event there is a desire for the facilities to change ownership, or there is a -name change of the
Permittee, a formal permit request must be submitted to the Division on official' Division form(s),
documentation from the parties involved, and other supporting materials as may be appropriate. The
approval of this request will be considered on its merits and may or may not be approved. The
Permittee of record shall remain fully responsible for compliance until a permit is issued to the new
owner.
6. The Permittee shall retain a set of approved plans and, specifications for the life of the facilities
permitted herein.
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority:
8. The Permittee must pay the annual fee within thirty (30) days after being billed by *the Division.
Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit
pursuant to 15A NCAC 02T .0105(e).

permit issued January 3, 2008 we are forwarding herewith a modified Attachment B, certified April 4,
2008, and updated location and soil maps. to the Town of Angier for the continued operation of the subject
wastewater treatment and surface irrigation facilities.
This certification is being issued to modify Attachment B to:
• Update field locations and identification numbers.
. ♦ Verify field acreage.
• Verify dominant soil series classification for each field including drainage, and erosion
classification.
• Establish annual loading rate for each field.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0002638 that was previously certified on January 3, 2008; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit: As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0002638, issued on January 3, 2008.
Please replace the existing Attachment B of Permit No. WQ0002638 that was previously
certified on January 3, 2008, with this modified Attachment B. Note that the vicinity, soils,
monitoring well location, and acreage maps .are attached to this modified Attachment B. It is the
Permittee's responsibility to ensure that a complete permit is created by inserting this modified
Attachment B and maps into Permit No. WQ0002638, issued on January 3, 2008, and making sure to
retain any vicinity and buffer maps for any existing and unmodified land application sites.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service

TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above.

I reviewed the application request document this morning for the Town of Angier (received in FRO on 23 Sept 2016). I
read over the information prepared by Coley Price and reviewed the Harnett County GIS website for the Angier
property. I've attached two maps from Harnett GIS that indicate that the parcels of property have been combined to
eliminate internal property lines to alleviate property line setbacks. The original staff report for this permit renewal was
submitted back in April. Do you need for me to prepare another staff report for the Town of Angier renewal based upon
the information received?

linear feet of 12-inch gravity sewer, a 450-GPM pump station; with dual 450-GPM pumps, approximately
620 linear feet of 8-inch force main, flow metering, an 8-acre facultative lagoon; with a natural clay liner.
having a hydraulic conductivity of 1 x 10-6 cm/sea or less, chlorination facilities, dual 700-GPM irrigation •
pumps with approximately 3,4601inear feet of 8-inch force main, a solid set spray -irrigation system which --
includes 36,000 linear feet of piping on a 46 acre sprayfield, approximately 1,585: linear feet of 2.5-inch
water line, approximately 970 linear feet of 2-inch water line, and all associated piping, . valves, controls,
and appurtenances :to serve the Town .of Angier,. with no discharge of wastes :.to the surface waters,
pursuant to the application received September 26, 2007,- and subsequent additional information received:
by the Division of Water Quality (Division), and _in conformity with the project plan, specifications, and .
other supporting data subsequently filed and approved by the Department of •Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until December 31, 2007, shall void Permit
No. WQ0002638 issued August 22, 2003, and shall be subject to the following specified conditions and •
limitations:
I. • SCHEDULES [please review the entire permit for reporting, monitoring, and other on -going activities]
1. Within 60. days of the permit issue date the Permittee shall submit to the -Division ail.updated site -
maps showing at a minimum the following information. • - : '
(1) Delineation of each irrigation field'clearly labeled with the Field identification number usedinn, :
Attachment B. If new nomenclature is required, indicate existing field identification numbe: and
new identification number. if.; ,
(2) Net acreage of each field
(3) Soil mapping unit and soil type of each field. .
(4) Anticipated maximum cumulative application rate (inches per year) for each field.
(5) Location of monitoring wells MW1, MW2, MW3, MW4, and MW5 •
2. lans and specifications for a flow measuring device pursuant to Condition IV. 2. shall: be prepared by
a licensed Professional Engineer and submitted to the appropriate regional office 30 days prior to

III. OPERATION AND MAINTENANCE REQUIREMENTS
1. The facilities shall be properly maintained and operated at all times. The ,facilities shall be effectively
maintained and operated as a non -discharge system- to prevent the -discharge of any wastewater :,
resulting from the operation of this facility. The' Permittee shall maintain an Operation and-:
Maintenance Plan pursuant to 15A NCAC 02T .0507: including operational functions, maintenance
schedules, safety measures, and a spill response plan.
2. Upon classification of the wastewater treatment and irrigation facilities by the Water Pollution
Control System Operators Certification Commission (WPCSOCC), the Permittee shall -designate and,
employ a certified operator to be in responsible charge (ORC) and one or more certified operator(s) to
be back-up ORC(s) of the facilities in accordance with 15A NCAC 08G .0200. The ORC shall visit •
the facilities in accordance with 15A NCAC 08G .0200 or as specified in.this permit and shall comply
with all other conditions specified in these rules. - :•
3. A suitable year round vegetative cover shall be maintained such that crop -health is optimized, allows
for even distribution of effluent, and allows inspection of the irrigation system::,
4. Adequate measures shall be taken to prevent wastewater ponding or runoff from -the irrigation field. ! •
5. Irrigation shall not be performed during inclement weather or when the ground is in a condition that ;
will cause ponding or runoff:
6. All waste application equipment must be tested and calibrated at least once per. year. Records of the. .-
calibration must be maintained for five years.
7. No type of wastewater other than that from Town of Angier shall be irrigated onto the irrigation area. -

5. The Permittee tracking the amount of wastewater disposed shall maintain adequate records. These
records shall include, but are not necessarily limited to, the following information: •
a. Date of irrigation, t:. k';
b. Volume of wastewater irrigated,
c. Field irrigated,
d. Length of time field is irrigated,
e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field,
f. Weather conditions,
g. Maintenance of cover crops,
h. Field number consistent with the soil type and application rate,and
i. Wetted area acreage for each individual field consistent with the soil type and application rate.
6. Freeboard (waste level to the lowest elevation on the top of the embankment) in the facultative lagoon
shall be recorded weekly. •
7. Monitor wells shall be sampled after construction and within 3 months prior to initiating waste
disposal operations. Monitor wells shall be sampled thereafter at ,the frequencies and for the
parameters specified in Attachment C. All mapping, well construction forms, well abandonment
forms, and monitoring data shall refer to the permit number and the well nomenclature as provided in
Attachment C. .
8. For the initial sampling of the well as specified elsewhere. in the permit, the perrnittee shall submit a
copy of the GW-1 Form (Well Construction Record) that lists this permit number and the appropriate
monitoring well identification number with, the Compliance Monitoring Form (GW-59) for that well.
Initial Compliance Monitoring Forms that do not include *copies, of. the GW-1 form are incomplete
and may be.returned to the permitteewithout being processed. • . .
9. Two (2) copies of the results of the sampling and analysis :must ,be received on Form GW-59
(Groundwater Quality Monitoring: Compliance Report Form); along with attached copies of
laboratory analyses, by the Division of Water Quality, Information Processing Unit, .1617 Mail
Service Center, Raleigh, North Carolina 27699-1617 on or beforethe`last workingday of the month
following the sampling month. Form GW-59 shall include the number of this permit and the
appropriate well identification number. One Form GW-59a certificationform shall be provided for
each set of sampling results submitted.
10_ Three (3) copies of all monitoring data [as specified in Conditions IV. 3. and IV. 4.] on Form NDMR
for each PPI and three (3) copies of all operation and disposal records [as _specified in Conditions IV.
5 and IV. 6.] on Form NDAR-1 for every field shall be submitted on or before the last day of the
following month. If no activities occurred during the monitoring. month, monitoring reports are. still
required documenting the absence of the activity. All information° shall be submitted to the following

11. An annual representative soils analysis (Standard Soil Fertility -Analysis) shall be conducted on each
irrigation field and the results maintained on file by the Pennitteefor a minimum- of five years.- The -
Standard Soil Fertility Analysis shall include, but is not• necessarily limited to, the following
parameters:

2. This permit shall. become voidable unless the facilities area constructed . in accordance• with4he
conditions of this permit, the approved plans and specifications, and other supporting data. 'i
3. This permit is effective only with respect to - the _nature- and volume of wastes .described. in .the
application and other supporting data. No variances lb'applicable'rules governing the construction
. and / or operation of the permitted facilities are granted unless specifically requested and granted in
this permit.
4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other government agencies (local, state,
and federal) that have jurisdiction. Of particular concern to the Division are applicable river buffer
rules in 15A NCAC 02B .0200, erosion and sedimentation control requirements in 15A NCAC
Chapter 4 and under the Division's General Permit NCG0I0000, and any requirements pertaining to
wetlands under 15A NCAC 02B .0200 and 02H .0500.
5. In the event there is a desire for the.facilities to change ownership, or there is a name change of the
Permittee, a formal permit request must be submitted to the Division on official' Division form(s),
documentation from the parties involved, and other supporting materials as may be appropriate. The
approval of this request will be considered on its merits and may or may not be approved. The
Permittee of record shall remain fully responsible for compliance until a permit is issued to the new
owner.
6. The Permittee shall retain a set of approved plans and. specifications for the life of the facilities
permitted herein.
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority
8. The Permittee must pay the annual fee within thirty (30) days after being billed by the Division.
Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit
pursuant to 15A NCAC 02T .0105(e).

This letter is in response to The Town of Angiers permit application return for the
Wastewater irrigation system. application No.. WQ000263 8.
Section A I. 1 have attached a copy of the set back waiver that, was provided by the
Division of Water Resources. The waiver has been signed and notarized by requested
property owners and recorded with the register of deeds in Harnett County. •
. Section A 2. • I - have also attached documentation that the three
parcels(0406839001,0406839002,0400839000) that made up our irrigation fields have
been combined into one parcel therefor eliminating propeity line setback issues through
out our irrigation fields.
Section A 3. In response to the wetted areas of the fields that where within 150 feet of
property lines irrigation heads have been capped, field acreage have been reduced, and
yearly loading has been reduced based on the current attachment B of our permit. The
new field acreage and yearly loading are as follows:
Field #1 4.23 acres 77.56in per year
Field ,#2 . 6.89 acres 113:88 in per year
Field #3 - 5.98 acres 38.39 in per year
,Field #4 -8.72 acres 52 in per year
Field #5 12.48 acres 47.43 in per Year
The Town of Angier believes that you have confused field 5F with field 3B there are no
surface waters within 100 feet of,field 5F. Field 3B has been capped off and is 'no longer ,
in service. The new acreage totals reflects this for field 3.

The Town of Angier also believes that you have confused field 5E with fields 5A and 5B.
The Town of Angier has removed field 5A form service and capped several heads in 5B
to meet our setbacks of 150 feet from property lines. The new acreage totals reflects this
for field 5.
The Town of Angier has reevaluated the setback distances to surface waters for fields 5E;
5D, 5C we are well within our setback limits to surface waters for these fields.
Section B. The Town of Angier has also provided proper documentation that proves
permit conditions I 1. and 1 2. from the current permit dated January 3, 2008 have been
meet.
Section F. A new site map has been provided showing new acreage of irrigation fields as
