Dear Mr. Morrison:
Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994
(or as subsequently amended).
The final permit includes a correction to the effluent monitoring requirements. Effluent
Fluoride monitoring was omitted in the draft permit but is now included in the final permit. A
letter notifying you of the correction was sent July 24, 2007. All the initial draft permit limits
and monitoring requirements are unchanged and are included in the final permit.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

requirements, and other conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective November 1, 2007.
This permit and authorization to discharge shall expire at midnight on May 31, 2012.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
The Town of Madison is hereby authorized to:
1. Continue to operate a drinking water treatment plant with a discharge of
decanted filter backwash and filter wastewater from dual settling lagoons. This
facility is located west of Madison on Lindsey Bridge Road at the Madison WTP in
Rockingham County.
2. Discharge from said treatment works at the location specified on the attached
map into an unnamed tributary to Big Beaver Island Creek, classified C waters
in the Roanoke River Basin.

A. (1.) EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS - FINAL
During the period beginning on the effective date of this permit and lasting
until expiration, the Permittee is authorized to discharge from outfall 001. Such
discharges shall be limited and monitored by the Permittee as specified below:

1. The discharge from this facility shall not cause turbidity in the receiving stream to exceed 50
NTU. If the instream turbidity exceeds 50 NTU due to natural background conditions, the
discharge cannot cause turbidity to increase in the receiving stream.
2. Upstream = at least 100 feet upstream from the outfall. Downstream = at least 100 feet
downstream from the outfall.
3. The pH shall not be less than 6.0 SU or higher than 9.0 SU.
4. The TRC limit will become effective May 1, 2009. Monitoring is required beginning November
1,2007.
5. The minimum measurement detection required is 0.5 mg/L. In the future a maximum limit of 1.8
mg/L will be imposed if data evaluation yields the potential to exceed 1.8 mg/L.
All samples collected should be from a representative discharge event.
There shall be no discharge of floating solids or visible foam in other than trace
amounts.

Mr. Morrison,
I was out of town all last week but have heard your voice messages and
read your email today. I did call back around 2:30 pm and left a voice
message.
I am working with David Russell from our Winston-Salem office to set up
a joint site visit at Madison as well as other facilities. I will
confirm the tentative time with you once I know. The answer to question
one is that wherever chlorinated water passes through the filter
(incoming raw water or the source water for filter backwash) it
automatically becomes a candidate for TRC limits because:
1) It is understood the water treatment plant is the control and source
of the chlorination.
2) The chemical residuals accumulate in the filter.
3) The residuals are flushed out with the backwash water.
4) Low levels of chlorine are toxic to aquatic life.
I believe question two can be better answered as part of our site visit.

In our phone conversation June 28, 2007 we discussed the flow schematic
for the Town of Madison Water Treatment Plant that is required to
complete the data for your submitted NPDES renewal application. Your
concern was that we wanted a formal diagram from an engineering source.
I explained that we did not need a formal engineering drawing but rather
just a simple block diagram indicating the direction of each water flow

(raw water, backwash, treated water, effluent water) through the various
treatment plant operations ( tanks, settling basins, filters, lagoons,
holding ponds, clarifiers, thickening tanks, drying ponds, etc) up to
the permitted outfall. The diagram should also clearly indicate where
the various chemicals used in the treatment plant entered the process
and indicate where the effluent sample is taken.
In most cases an operator at the water treatment plant is able to sketch
out a flow drawing and properly label the components that meets this
requirement. Please advise when this will be completed, it is important
that all the requirements of the application are met.

Mr. Morrison,
The schematic you provided is excellent. I do need to clarify a few
items on the schematic.
1) In the "Post Filter Chemical Injection" what chemicals are added (for
example chlorine, fluoride, caustic, ammonia, etc.)?
2) What is the source of the "Backwash Water"? Is it raw water with no
chemical additions or is a side stream from the incoming raw water that
is already chemically treated?
I assume based on the schematic the backwash water is not "Treated
Water". To address you question about removing the TRC limit (chlorine)
limit the actual source of the backwash is required. Therefore, question
2 above needs to be correctly defined.
I received an inquiry from you plant operator along the same lines. He
had an additional concern about measuring precision and how to meet the
TRC limit. I pointed out the 18 months extension from the permit
effective date listed in the footnote is there to allow you time to
resolve these issues. My understanding from the inquiry conversation is
the backwash water is a side stream that has the chemicals added. Since
you were previously required to measure and report TRC in the effluent
in your existing permit then I have to assume chlorine is added to the
backwash water at some point. Until TRC data is measured and reported at
the corrected precision level its true impact is unknown.

I had left David a vm yesterday to call me to discuss Madison WTP operation in lieu of
the recent information which indicated source of backwash water is post treated water.
Explained the history of resolving the renewal application missing information and how
we have received a flow schematic and other data. Current problem to resolve on permit
draft was whether fluoride was in the backwash water source. Asked David to contact
Madison WTP and confirm source of backwash water. David made contact, called
back, and received confirmation that drinking water is source.
Since this is the source then fluoride monitoring will need to be added.
SUPPLIMENTAL INFORMATION:
Discussed addition of Fluoride Monitoring to permit with Gil and Jim. Monitoring only is
applicable and does not require public notice. Focus should be on collecting data for
future evaluation.

Mr. Morrison,
You should have received a copy of the proposed permit NPDES draft for the Madison
Water Treatment Plant. To continue finalizing your permit we need you to submit a
flow schematic of your water treatment process including showing what and where
chemicals are added. It should start with the incoming raw water and terminate at the
permitted outfall. Also, please indicate where samples are collected for the DMRs.
This is part of the application process and is actually found on page 3 of the
application under Item 15 (for some reason your application page 3 shows this as Item
2). A clearly drawn and labeled hand sketch is fine. You can mail or fax or email the
sketch.
Your assistance in taking care of this missing information is greatly appreciated. If
you have any questions let me know.

Solids accumulated in the alum sludge lagoons are dewatered by inserting well points in the out of service
lagoon, liquid is then pumped to the other basin.
After the solids are dewatered, the solids are transported to town property for land application as per land
application permit #WQ00019753.
