Dear Mr. Stabler:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
The terms included in this final permit are considerably different from those found in draft permit sent to
you on July 15, 2009. Since that time, this office has reevaluated its guidance for the permitting of
wastewater discharges from conventional water treatment plants. The results of that reevaluation are
included in the permit. While some parameters have been removed from the list of those to be
monitored and others have been added, the overall effect of these changes is believed to impose less of a
regulatory burden to the permittee. We would like to thank Mr. Ron Farnsworth of your staff for his
assistance and persistence during the permitting process.
The changes between this final permit and the original draft were described in an October 15, 2009 e-
mail from Bob Sledge of our staff to you and Mr. Farnsworth. Changes in this final version include:
• The flow limit has been removed. The monitoring requirement for flow has been changed from
weekly, instantaneous measurement to continuous flow monitoring, based upon a more complete
understanding of the WTP's operations. However, the requirement for continuous flow monitoring
of the effluent will not become effective until March 1, 2011. This should allow the Town time for
planning, budgeting and installation of an adequate flow measurement device.
• Monitoring for Calcium and Magnesium have been eliminated from the permit. Monitoring for
ammonia nitrogen has been removed because the WTP does not use chloramines.

• Monitoring for Turbidity has been reestablished as a term of the permit, but monitoring of the
effluent (as opposed to instream monitoring) is now to be performed.
• Monitoring for most constituent parameters has been reduced to quarterly.
• Quarterly monitoring for Total Nitrogen and Total Phosphorous have been added to the permit.
• Footnote 3 from the draft permit has been removed. There is no explicit provision within this permit
to have monitoring for particular parameters reduced or removed during the life of the permit based
upon submittal of data from the discharge. Such modification may be requested, but would be
deemed a major modification of the permit and subject to (currently) a $260.00 modification fee.
Otherwise, monitoring frequencies shall remain the same through the full term of the permit. At the
time of the next permit renewal, monitoring parameters, frequencies and limits could be changed
(added, reduced or eliminated) based upon results of sampling during this permit cycle.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

1. The requirement for continuous flow monitoring will become effective on March 1, 2011.
Until that date, flow monitoring shall be conducted on a weekly basis, using instantaneous
sampling methods.
2. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported by
a North Carolina certified laboratory (including field certified), even if these values fall below 50
µme•
3. The pH shall not be less than 6.0 standard units, nor greater than 9.0 standard units.
4. Monitoring should be performed in conjunction with toxicity testing.
5. See Condition A. (2.) for toxicity testing requirements. Toxicity sampling should coincide with
sampling for parameters covered by footnote 4.
All samples collected should be of a representative discharge.
There shall be no floating solids or foam in other than trace amounts.

A. (2.) CHRONIC TOXICITY MONITORING (QUARTERLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90 %. The testing
shall be performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during
the months of February, May, August and November. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility name, permit number, pipe number, county, and the month/year of the
report with the notation of "No Flow" in the comment area of the form. The report shall be submitted
to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later
than the last day of the month following the month of the initial monitoring.

I'm sorry it's taken so long to bring about more movement on the issuance of this permit. There has been a good deal of
reevaluation of our current permitting strategy for WTPs over the past few months. Those who have conducted the review
will soon issue a modified strategy to our staff for development of new permits. Because of the incomplete status of some
of the permit renewals I'm handling, I've been authorized to offer the terms of the modified strategy to those permittees.
While there may be parts of this strategy that some people will remain dissatisfied with, I believe that overall, it is an
improvement from what we were using in the past.
Please be advised that for the combined sake of consistency and management of the workload in this office, I can offer you
essentially the terms you saw in the draft permit, or those noted within this draft version of a modified permitting strategy
for conventional water treatment plant discharges. Changes within the new strategy include:
The new strategy contains no flow limit. However, flow -based monitoring requirements and limits will be determined
through reexamination of flow data already provided.
Monitoring for most constituent parameters will be quarterly.
Monitoring requirements for Calcium and Magnesium have been eliminated.
Turbidity is reestablished as a monitored parameter (as it is in the current permit).
There is no explicit provision under this strategy to have monitoring for particular parameters reduced or removed during
the life of the permit based upon submittal of data from the discharge. Such modification may be requested, but would be
deemed a major modification of the permit and subject to a $260.00 modification fee. Otherwise, monitoring frequencies
shall remain the same through the full term of the permit. At the time of the next permit renewal, monitoring parameters,
frequencies and limits could be changed (added, reduced or eliminated) based upon results of sampling during this permit
cycle.
A copy of a proposed monitoring and limits page for the permit is attached. Please note for your particular permit that
effluent monitoring for Total Nitrogen and Total Phosphorous have been added. These parameters were not included in
the draft permit due to an omission on my part. Monitoring for these parameters would have to be included in either
version you would choose.
Under either version, the flow limit will be removed. However, the requirement for continuous flow monitoring will
remain. To allow you time to budget, plan, and install an adequate flow meter, this requirement will not take effect until
March 1, 2011.
It would appear to me that the new strategy option would be the better deal for most WTPs. If you would like to talk about
it, please give me a call or reply to this e-mail. I will be in the office until 11:30am this morning and until 2:30pm
tomorrow (Friday).

First of all, I want to apologize for the late response on these NPDES draft permits. Even though we have
discussed my concerns via phone conversation, I should have provided you a written response before now.
Tom Belnick and I also discussed my concerns via a phone conversation on June 19, 2009.
As you know, my primary concerns are the proposed monitoring changes for these Water Treatment Plant
(WTP) discharges. If there are true environmental concerns associated with these longstanding discharges, then
certainly the concerns should be addressed. However, especially in view of current economic difficulties for
most local governments, added monitoring requirements, without sound justification, seems inappropriate.
Many of the added constituents proposed for monitoring should not be a significant presence in the WTP
discharges. These appear to include: calcium, magnesium, fluoride, copper, zinc, and ammonia nitrogen.
Corrosion inhibitors, fluoridation, and chloramination are all introduced after filtration at a WTP. Therefore, it
is very unlikely that associated contaminants would ever be in backwash water, or settled water sludge.
Additionally, I would like to see Whole Effluent Toxicity monitoring quickly removed from the monitoring
requirements, if no problems are observed.
It seems strange to me that monitoring requirements for Settleable Solids and Turbidity have been removed
from these permits. These should be the main constituents in conventional WTP discharges, and permittees
need to be aware to keep these contaminants to a minimum in their discharges.

to South Potts Creek in the Yadkin Pee -Dee River
Basin.
The Aqua North Carolina, Inc requested a new per-
mit N00088854 for Pine Lakes Subdivision Well # 2
in Surry County; this facility discharge is treated
filter backwash to unnamed tributary to Stewart
Creek, Yadkin River Basin.
The Town of Yadklnvfle requested renewal of per-
mit NC0079260 for the Yadklnville WTP in Yadkin
County. This permitted discharge is treated filter -
backwash wastewater to South Deep Creek in the
Yadkin -Pee Dee River Basin.

7. Describe the treatment process(es) for raw water.
Water is pumped from the Yadkin river into our three storage ponds going to our upper
lake first and then released to our lower ponds giving the longest settling time possible.
Then the water is pumped from our off stream storage ponds to the plant where we add
ferric sulfate to it for our coagulation process. Sodium permanganate is also added for
oxidation as well as chlorine for disinfection. After coagulation and sedimentation the
water is filtered through our 20 mixed media filters. After filtration, we add additional
chlorine for disinfection, fluoride for prevention of dental cavities, lime for PH
stabilization and zinc orthophosphate for corrosion control.
8. Describe the wastewater and the treatment process(es) for wastewater generated
by the facility.
Wastewater from the treatment process flows to two 350,000 gallon waste ponds for
settling. The decant water is allowed to return to the Yadkin River after it has been
dechlorinated using calcium thiosulfate.
15. Solids Handling Plan
All backwash water, water from track -vac system and settling basins wastewater goes
into our two sludge ponds. Water is decanted off the sludge ponds leaving the sediment.
The sediment is pumped off the bottom of the sludge ponds onto two vacuum beds where
water is sucked off leaving the sludge to dry and then being taken to the landfill. We
have a land use permit for our sludge and when our vacuum beds won't keep up we have
the sludge land applied. This occurs about twice a year.
