Facility and Address: Facility: City of Burlington's Lake Mackintosh WTP, 3400 Harris Road, Burlington, NC Mailing Address: Mr. W.R. Baker, City Manager, City of Burlington, P.O. Box 1458, Burlington, NC 27216 2 Date of On -Site Investigation: March 8, 1994. On site inspection conducted with Tom Muir -Chief Operator. 3. Report Prepared by: Arthur R. Hagstrom, Environmental Engineer, DEM, WSRO, WQ. 4. Persons Contacted and Telephone Number: A. D. Directions to the site: Exit I-40/85 at Elon College Exit and go south on Huffman Mill Road (3 1/2 +/- miles) and turn left on Harris Road and go to the WTP. 6. Topography (relationship to the 100 year flood plain included): In front of dam for the lake for the discharge and on graded area at near top for the input. Input is not in flood prone area. Watershed classification: Class "C-NSW " b. River Basin and Subbasin: Cape Fear/ 03-06-03 c. Discharges of supernatant will go directly into the surface water. The downstream goes to the Haw River south of the Burlington area. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. The existing WTP has to dispose of supernatant from its sludge basins. This new transfer system will provide an appropriate way. a. What is the current permitted capacity of the facility? 450,000 GPD (Ultimate Design Capacity) +/- NOL b.The daily discharge flow from the new lagoons for the WTP is estimated at.450 MGD+/-. When this A to C is completed. c. Date(s) and construction activities allowed by previous Authorizations to Construct issued in the previous two (2) years: N/A d.What is the actual treatment capacity of the current facility design? .450 MGD per design from sludge from back washing filters and washdown of settling basins. d.Please provide a description of existing or substantially constructed wastewater treatment facilities: The WTP is in need of the tansfer system that will be provided by this A to C. The system will pump or gravity drain the supernatant to an existing surface storm drainage system. 2.Please provide a description of proposed wastewater treatment facilities: Add piping, valves, and pumps to enable discharge of supernatant.
