Just prior to reaching I-85, turn right onto NCSR 1138. Proceed and then bear left onto NCSR 1290. Hill Top Rest Home is located at the end of the road. 6. Discharge Point(s), List for all discharge points: Latitude: 35° 43' 39" Longitude: 80° 23' 05" U.S.G.S. Quad No. E-17NW U.S.G.S. Quad Name Salisbury 7. Site size and expansion area consistent with application? There is adequate land available for upgrading or expansion. 8. Topography (relationship to flood plain included): The facility is located well above the flood plain. 9. Describe receiving stream features and pertinent downstream uses: The receiving stream is basically a dry ditch that discharges into a swampy area of High Rock Lake. Part II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. a. Volume of Wastewater to be permitted: 0.003 MGD (Ultimate Design Capacity) b. What is the current permitted capacity of the Waste Water Treatment facility? 0.003 MGD c. Actual treatment capacity of the current facility (current design capacity)? 0.003 MGD d. Please provide a description of existing or substantially constructed wastewater treatment facilities; The facility currently consists of two (2) septic tanks, two (2) flow splitter valves (can divert flow between nitrification line systems and sandfilter), (1) grease trap for kitchen wastes, two (2) pump tanks with 80 gpm pumps, alarm system, two (2) surface sand beds, a recirculation diversion box, tablet chlorinator, contact tank, flow measuring device and cascade aeration steps. e.
