Subject: Waiver of Monthly Reporting Requirement
NPDES Permit NC0028169
Stalite / Aquadale Quarry
Duplin County
The Division has reviewed your request to suspend submission of monthly Discharge Monitoring Reports
(DMRs).
After considering the recommendations of the staff in the Mooresville Regional Office, I have agreed to
waive the requirement to submit monthly DMRs for the subject facility, effective December 1, 2011. The
DMRs up through (and including) November 2011 should be submitted as usual.
Be advised that if and when any wastewater discharge occurs, the discharge must be monitored as per the
terms of the NPDES permit and other applicable rules or statutes. Please keep the Mooresville Regional
Office apprised of any pertinent developments that arise regarding this facility, especially if the discharge
is re -activated.
While requirements for operator visitation, monitoring and reporting are being waived, you are still
required to pay the Annual Administering and Compliance Monitoring Fee that is associated with this and
all NPDES permits.

Beginning with the permit effective date and lasting until expiration, the Permittee is authorized to discharge

1. Sample Locations: E — effluent; I — Influent; D = downstream, — 100 feet below the outfall.
2. The Permittee shall record the flow rate and duration of each discharge.
3. Per Batch: The Permittee shall confirm permit limit compliance per batch by laboratory analyses prior to
discharge or mixing. Batches shall be numbered, time -identified and dated. The Permittee shall keep separate
and contained onsite, each treated batch until the absence of toxicity is confirmed by certified lab analysis.
Onsite batch storage shall accommodate a minimum turn -a -round time for WET testing (from sampling to lab
reporting).
4. Total PAHs: Permit limit applies to the sum total of all (seven) poly -nuclear aromatic hydrocarbons (PAHs)
to include Chrysene, Benzo-a-anthracene, Benzo-b fluoranthene, Benzo-k-fluoranthene, Benzo-a-pyrene,
Ideno(1,2,3-cd)-pyrene, Dibenzo(a,h)anthracene.
5. Phenolics — compliance with permit limits shall include 2 of 3 test methods. [See §A. (5.)].
6. Whole Effluent Toxicity (WET): Pass/Fail Static 48-hour (Acute) @ 0.22 %; [See §A. (4.)].
Units: MGD = million gallons per day µglL = micrograms per liter
NTU = nephelometric turbidity units mg/L = milligrams per liter
Conditions: The Permittee shall discharge no floating solids or foam.

Dear Mr. Weaver:
I would like to request a DMR waiver for NPDES permit NC0028169. Stalite,
the light weight aggregate produce, has temporarily stopped processing at the Aquadale
Quarry location due to the economy. They are no longer burning or processing light
weight aggregate. Aqudale Quarry will continue to process raw material and comply
with our NPDES permit NCG070006. When light weight production resumes we will
then operate under and comply with NPDES permit NC0028169 and resume DMR
submittals according to the permit requirements.
