Everything looks good except for a few small issues: Please correct the following: - Supplement-EZ Form, Wet Pond Page, Wet Pond 2 Column, Line 32 – The forebay volume shown for this item (29,611 cf) does not correspond to the volume shown in the provided calculations (29,632 cf) NOTE: This may have been an oversight on my part during the original permit review since it is noted that the wet pond design and calculations have not changed as a part of this major modification. - Please provide proof that the O&M Agreement Forms for this project have been recorded with the Register of Deeds and that they have been recorded on the plats for the site. This item is required per General MDC 11(15A NCAC 02H .1050(11)). The plats that were provided in the submission appear to have the necessary language (Note number 18 or 19 depending on the plat), but they do not reference the O&M Agreement deed book and page number so these plats will need to be re-recorded (and the O&M Agreements will need to be recorded if not already done so). - While not required, it is recommended to increase the elevation of the bypass weir for Wet Pond 2. Currently, the bypass weir has an invert of 587.75 ft and the temporary ponding surface elevation from the design volume is 587.81. This configuration is not specifically against the MDCs, but it is against the spirit of the MDCs since the storage volume stored above elevation 587.75 is quickly drawn down via the weir instead of slowly drawn down via the orifice over the 2-5 day period required per Wet Pond MDC 7. Please provide the following: - 1x hard copy & 1x electronic copy of the revised page of the Supplement-EZ Form. - 1x hard copy & 1x electronic copy of proof that the O&M Agreement Forms have been recorded with the Register of Deeds. - 1x hard copy & 1x electronic copy of proof that the plats reference the O&M Agreement Forms. - 1x hard copy (2x hard copies for plans) & 1x electronic copy of any other items that need to be modified in order to address these comments. Reviews under the standard permitting process generally take 30 – 45 calendar days, but can take up to 90 3 calendar days per statute (our other reviewer, Corey Anen, no longer works for the State, so this review will probably be closer to 45 days than 30). At this time, the submission is not administratively complete and cannot be accepted for review in its current condition. Please provide the following submittal items by close of business on 2/17/2022 so that we may accept the project and begin the review process, otherwise, the project will be returned as administratively incomplete. - 1x original, signed designer’s certification forms. It is noted that the wet pond specific form was provided in the latest submission, but the other two forms, the “As-built Permittee Certification” and “As-built General MDC Certification” were not provided and are required. NOTE: The website states that these forms are always required (they were also provided as part of the final permit package alongside the wet pond as-built 5 certification form). Like with the wet pond form, complete as much of the form as you can since this is a partial certification. If these items are received within the stipulated timeframe, we can accept the project and begin the review process.
