The attached PCN is the buffer authorization application. (Highlighted in yellow are items
within the application that need to be filled out).
After you fill out the application send three copies of the Buffer Authorization Application to

Below are listed some key general points that are needed for buffer authorizations that are
sometimes missed in applications:
* You can fill out this form but the owner needs to sign where indicated because the permit
will go the them. Or fill out an agent authorization form (example attached)
* Site plan with the proposed permanent impacts indicated
* Any temporary impacts beyond the permanent ones that are needed for construction. How
these will be stabilized and replanted if necessary.
* Stream indicated on the plan
* Buffers on the plan, Zone 1 - 30 feet from top of the stream bank; Zone 2 - from 30 feet to 50
feet. Paralleling the stream pattern.
* Try to keep all new impacts in Zone 2 if possible
* Amount, square footage, of impacts.
* How will the rainfall on the new impervious surface be directed or handled - For
example: The stormwater from the proposed area may flow from the proposed impervious area
as sheet flow. We want to avoid piping any run -off through the buffer and directing directly to
the stream.
* Describe the purpose and need of the impacts (explain why the buffer impacts are necessary)
* Avoidance & Minimization? Explain how the buffer impacts are limited to .... And only to
zone 2 ... etc.
