Dear Mr. Smith,
I am attaching a copy of a May 3, 2014 email you sent to Chris White per the proposed herbicide spraying of the
Wilson Industrial Airport Runway 33 approach area.
Enclosed is a completed PCN form I have filled out for this project as Applicant for the City of Wilson, the owner of
the Wilson Industrial Air Center. I have attached numerous supporting documents.
A brief explanation of this project is that vegetation in the wooded sections of Runway 33 were an obstruction and
hazard to aviation. This vegetation was to be removed per a letter (attached) sent to Gronna Jones, Airport Manager,
by Airport Inspector James Capps of the NC Dept of Transportation, under authority from the Federal Aviation
Administration. These trees were mostly cut in the Fall of 2013 by a contracted logging operation (most of the trees
were just cut and felled and left). However, there were certain trees just under the height minimums that could not
be cut in the the logging operation, due to the fact they were growing directly in, on, or next to, a man-made ditch
shown as a blue line stream on USGS topo maps. The logger could not cut them this past Fall because his
equipment could not reach them, and, he felt it was unsafe to put a man with a chain saw into the ditch and try to
reach high to cut a 6 foot tall stump. Within just a few years, these trees will thus be tall enough to be a hazard to
aviation themselves and the City will be faced with having to somehow find a way to cut and fell these trees. The
reason these trees are in/on the ditch is neglect of maintenance to keep the ditch clear of vegetation when the trees
were smaller and could just be mowed.
The safest and most direct way to handle the trees growing in this `blue line' ditch, as well as the `cone' dictated by
the DOT to be cleared otherwise, is to herbicide spray this entire approach area with an aerial spray that is labelled
for use in aquatic areas. This herbicide prescription is approved by Don Johnson (see attached document), a licensed
Pesticide Consultant, and is proposed to be sprayed by Greenwood Helicopters, a licensed applicator. Chris White
of Red River Specialties is the contracting party for the actual spraying.

I recently inspected the Wilson Industrial AiLf cater as part of the FAA 5010 Airport
Master`Record updateiprocess. Enclosed is a set'of,photographs taken during the inspection of
the airport,,some of which, depict.potential,safety concerns mentioned Below. Also, enclosed is a
marked -up version of the 5010 form, showing the changes to be electronically submitted to the
FAA. Based on my discussions with Steve Moore -who was working at the FBO at the time of
my inspection, and based on the information you emailed to me, there were minimal updates to
the Master Record.
As part of the 5010 update process, we are required to inform the airport manager or
owner, of any items that might compromise safety or may not meet federal standards. With
regard to the FAA criteria for this ai_mort. I offer the following:
• With non,- precision approaches, the recommended distance is 200 feet for hold lines
from the runway centerline. The hold dines at the FBO, both sides of Runway 27, both
sides of Runway 33, at Runway 15, and at Runway 9 are less than this recommended
distance. For non - perpendicular hold lines, the closest point on the line is measured to
the runway'centerline.
• The box marked for emergency shut off at the fuel farm is locked and there ,are no
instructions for operation.of this emergency mechanism.
• The warning placards with the indications of flammable, no smoking, and type of fuel
need to be refreshed and visible on all sides of the tanks at the fuel farm, including the
smaller tank outside of the containment wall.
• The,second taxiway light from Runway 27`was broken.
• The sole taxiway light at the hangers'wasnot operating at the time of inspection.

• The third runway light from the FBO taxiway was not operating at the time of inspection.
• The second runway light on Runway 3/21 from the intersection with Runway 15/33 was
not operating at the time of inspection.
• On Runway 3121, the lights on both sides of the runway in the vicinity of the FBO
taxiway were not operating at the time of inspection.
• On Runway 3 the two outer end lights on the right side were not operating at the time of
inspection.
• On Runway 3, one light in the outer PAPI box was not operating at the time of
inspection.
• The aiming angle of the PAPIs should be periodically checked and a log maintained
verifying those inspections.
• There was some sediment residue on certain portions of the runways indicating possible
ponding concerns. There was standing water on Runway 3/21
• At Runway'33, there is severe ground erosion around the catch basin creating a hazard in
the safety area.
• Runway 15/33 has longitudinal/transverse cracking, as well as block.pavement cracking
with loose debris creating,a POD hazard.
• On Runway 1'5/33 the crack sealant is failing.
• There were two PK nails set at Runway 27 and one needs to be removed.
• The markings are cracked and discolored on Runway 3121 and on Runway 15/33
• There is vegetation in the pavement of Runway 9/27 and Runway 15/33.
• The FAA's standard runway lighting for a non - precision approach runway requires four
end1ights on each,side and there,are only threeend' lights on each side for Runway 3/21.
• Some of the taxiway lights, particularly between Runway 27 and Runway 21, have
exposed angle irons, which are,non- frangible and should be at or below grade.
• It is a requirement that the airport manager notify the Raleigh Flight'Service Station of
any activities-on,or in the'vicinity of the runways to establish a local,NOTAM during the
time of these activities. At the time of the inspection, I noted the following, activities
which may be.currently conducted at the airport that atisuch time of the activities'would
require that a NOTAM be issued by the airport manager:
• the lacy enforcement vehicular driving course conducted on the runway(s);
• the location and operation of equipment in the runway approaches, including-the
backhoe currently parked off of Runway 15.
• As you mentioned, the crack sealant performed 6n,Runway'9 was excessive, creating a
build -up on the runway and uneven surface. Some of-the crack seal has loosened
creating loose debris and leaving the original cracks exposed.
• Runway 9 has a short line painted below the runway number which is not in accordance
with FAA standards.
• For Runways 9 and 15, the beginning thresholds for these runways are not correctly
established. The marking advisory circular dictates how adjoining runways should be
marked (for ,example, one runway end starts at the edge of pavement and the other
runway end woufd be displaced — including a threshold bar - to begin after the
intersection of the two runways).
• The approach to Runway 3 is 15 :1 due to 31' +/- trees located approximately 683' from
the end of runway and 242' right of the centerline. (pilot's approach view).
• The approach to Runway 21 is 26:1 due to 14' +/- trees located approximately 570' from
the end of runway and 76" left of the centerline. (pilot's approach vlenv).
• The approach to runway 9 is 14:1 due to 19' +/- trees located approximately 483' from
the end of runway and 58' right of the centerline. (pilot's approach vie►v).

• There are close -in obstructions to Runway 9 due to 5' +1- brush located approximately
46' +/- from the end of'the runway and 135' right of the centerline (pilot's approach
vie►v).
• The japproach to Runway 27 is 7:1 due to 26' +/- trees located approximately 393 from
the end of runway and 229' right of the. centerline. (pilot's, approach view).
• The approach to Runway 15 is 20:1 due to,41' +/- trees located,approximately 1022' from
the end of runway and 12' left of the centerline. (pilot's approach vieiv).
• There are close -in obstructions to Runway 15 due to 8' +/- trees located approximately
93' +/- from the, end of the runway and 145' left of the centerline (pilot's approach vie►v).
• The approach to Runway 33 is 3:1 due,to 39' +1- trees'1 ca e '

lbe,end'of runway and 184' right of the centerline. (pilot's approach view).
• The existing 110 cautionary remarks listed in the Airport Master Record were still
applicable at the time of this inspection
Please note that the FAA has stated that airports with RNAV approaches must
1 Slone on those apnroaches and should any have penetrations causina slopes_ o
FAA,has instructed that these obstructions'need to be addressed in a,timelyUanner or
, nip-ht time operations to the penetrated runway may be disallowed. All',of the runways hamt
RNAV approaches, with the exception of Runway 27. Please remember that the obstructions,
identified during this inspection are based on CFR 14 Part 77 which defines that the beginning of
the approach slope starts at a point 200 feet beyond the pavement end. Any clearing,to maintain
these slopes should be in accordance,with approach, information obtained from a registered land
surveyor and/or professional engineer.
In accordance with recommended FAA SOIO airport safety data inspection procedures,
the above measurements are based ,on dimensions taken from visual observations using ,non -
precision instrumentation in the, field for obstructions in the approach. These,services are not to
be considered or relied on as professional engineering and/or surveying services.
