Please advise the facility's Operator -in -Responsible Charge (ORC) of our findings by forwarding the enclosed report. As discussed in the inspection report, a new ORC was identified on the October 2021 monitoring report but has not been officially designated by you as required by 15A North Carolina Administrative Code (NCAC) 08G .0201 and subject NPDES Permit. This Office requests that the new ORC and any new Backup ORCs be designated on the approved documentation and submitted to the Division within fourteen (14) days of letter receipt. The permittee must ensure the building housing the wastewater treatment units is properly locked/secured. The wastewater facility has not been in operation since June 2021 due to no influent flows. It is recommended that the permittee submit a request to the Division for an inactive status designation if there are no future plans to have tenants in the houses. No effluent limit violations were reported. The December 2021 and January through March 2022 eDMRs were submitted late. The Division has previously addressed the late submittal of eDMRs through the issuances of a Notice of Deficiency, Notice of Violation (NOV) and NOV/civil penalty assessments. The ORC and/or Backup ORC visitations were not documented on the August 2021 eDMR. A revised August 2021 eDMR will resubmitted. The ORC and staff must ensure all visitations are performed and documented. Comment: According to the Permittee, only one house has been connected to the wastewater treatment plant and there have been no occupants in that house for approximately one year. The ORC continues to visit the facility weekly; however, the wastewater system was shut down in June 2021. Only a small amount of water/wastewater was observed in both the RBC unit and small lift station with no evidence of previous overflows, discharges, etc. being observed during the inspection. Effluent Pipe Is right of way to the outfall properly maintained? Are the receiving water free of foam other than trace amounts and other debris?
