Facility discharges to stream with 7Q10/30Q2 = 0. Removal of discharge is recommended in
three years or new limits (page 2) will apply at that time. Facility must submit an engineering
report within 12 months of the effective date of this permit showing no alternative to surface water
discharge. instream monitoring has been done for this facility. If this facility is to be closed
within the year instream monitoring would not be necessary, however, if there is to be continued
discharge for a longer period technical support strongly recommends instream monitoring.

A letter of request for Authorization to Construct was received April 2,
1984, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 0.00375 MGD wastewater treatment facility consisting
of a new grease trap, an existing septic tank, an existing dosing tank, upgrade
the existing sand filter, chlorination and post aeration to serve the Bethany
School.
This Authorization to Construct is issued in accordance with Part III para-
graph C of NPDES Permit No. NC0037001 issued June 15, 1983, and shall be subject
to revocation unless the wastewater treatment facilities are constructed in accor-
dance with the conditions and limitations specified in Permit No, NC0037001.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston Salem Regional Office, telephone number 919/761-2351 shall be.
notified at least twenty-four (24) hours in advance of backfilling of the installed
surface filter system so that an in -place inspection can be made of said system
prior to backfilling. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:00 A.M. until 5:00 P.M. on Monday through
Friday, excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting it's
NPDES permit effluent limits, Rockingham County Schools shall take such immedi-
ate corrective action as may be required by this Division, including the con-
struction of additional wastewater treatment and disposal facilities.
The sand media of the surface filter must comply with the Division's sand
specifications and must be analyzed and approved by this Division either by
direct sampling or by acquisition of filter sand from a dealer who is currently
certified by the Division as an acceptable source.
One (1) set of approved plans and specifications is being forwarded to
you. If you have any questions or need additional information, please contact
