There are still some inconsistencies with the PCN that need to be corrected for me to continue processing the
application for DWR project # 20140550
Spuntech Industries LTD (as originally submitted in Part A -3 of the PCN, and now submitted in part A -4 of the corrected
PCN) is not listed on the NC Department of the Secretary of State's Corporations website
(http / /www secretary state nc us /Corporations /CSearch aspx) If you wish for Spuntech Industries LTD to remain the
applicant, they must be a company listed with the NC Secretary of State's as a corporation current - active status Further,
the agent authorization form would need to correspond, so would also need to be updated to read Spuntech Industries
LTD and be signed by the proper representative
The agent authorization form that was included with the original submittal was signed by Ron Bahat with Spuntech
Industries, Inc Spuntech Industries Inc is listed on the Secretary of State's website as current/ active status If you wish
for the applicant to be Spuntech industries, with Ron Bahat as the primarily responsible individual, then please update

Part A -4 on page 2 of the PCN to reflect this The current agent authorization form would not need to be updated in this
case
