Dear Mr. and Mrs. Oakley:
On June 17, 2014, the Division of Water Resources (Division) received your application requesting
a 401 Water Quality Certification from the Division for the subject project. The Division has
determined that your application is incomplete and cannot be processed. The application is on-
hold until all of the following information is received:
The Division is unable to accept your application without essential information currently
missing from your Pre - Construction Notification (PCN) Form. Please complete the
following Sections of the PCN Form and submit four (4) copies of this documentation:
• Section B — 3 Project Description (page 3 of 10)
• Section C — Proposed Impacts Inventory (pages 4 and 5 of 10)
• Section D­ Impact Justification and Mitigation (page 6 of`10)
• Section F —1 Environmental Documentation (page 9 of 10)
• Section F­ 3 Cumulative Impacts (page 9 of 10)
2. The PCN Form was not signed. The PCN form must be signed by the owner in order for the
Division to process your application. If an agent of the owner signs the application form,
also include an agent authorization form from the applicant. An example agent
authorization form can be found online:

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of•the above requested
information for the proper consideration of °the application. If all of the requested information is
not received in writing at the, address�below within 30 calendar days of receipt of this letter, the
