Before submitting this form, please ensure you have submitted the Pre -Filing Meeting Request Form as
DWR will not be able to accept your application without this important first step. The Pre -Filing Meeting
Request Form is used to satisfy 40 C.F.R. Section 121.4(a) which states "At least 30 days prior to
submitting a certification request, the project proponent shall request a pre -filing meeting with the certifying
agency." In accordance with 40 C.F.R. Section 121.5(b)(7), and (c)(5), all certification requests must
include documentation that a pre -filing meeting request was submitted to the certifying authority at least 30
days prior to submitting the certification request.

able to accept your application without this important first step. The Pre -Filing Meeting Request Form is used in
accordance with 40 C.F.R. Section 121.4(a) "At least 30 days prior to submitting a certification request, the project
proponent shall request a pre -filing meeting with the certifying agency" and in accordance with 40 C.F.R. Section
121.5(b)(7), and (c)(5) all certification requests shall include documentation that a pre -filing meeting request was

C. Project Information and Prior Project History
C.4a. Describe the existing conditions on the site and the general land use in the vicinity of the project
at the time of this application:
The project area encompasses the US 220/US 311/NC 135 interchange and approximately 1000 feet of
US 311/NC 135 on the west and 1000 feet on the east sides of the interchange. Land cover within the
project area is primarily comprised of the road surface and maintained ROW landscape with some small,
forested areas. The forested areas are predominantly Dry-Mesic Oak -Hickory Forest, although there is
an area of Piedmont Headwater Stream Forest along the north-east edge of the project area. Land use
within the project area is predominantly transportation. There are several commercial areas that are
adjacent to the project area including Lowe's Home Improvement, Wal-Mart, AutoZone, and multiple
restaurants. The project limits cross the access to these areas.
C.4h. Explain the purpose of the proposed project:
The North Carolina Department of Transportation (NCDOT) proposes to improve future 1-73/US 220 at
the US 220/US 311/NC 135 interchange in Rockingham County (STIP No. 1-5898). The proposed project
will replace Bridge No. 780074 and upgrade the interchange to accommodate future 1-73 improvements.
The purpose of the project is to improve traffic flow and operational deficiencies within the interchange
area to ensure an acceptable level of performance as development and traffic continue to increase in
the area.
C.4i. Describe the overall project in detail, including the type of equipment to be used:
The project will replace Bridge No. 780074 over US 220 and upgrade the interchange to accommodate
future 1-73 improvements. The proposed bridge will be aligned north of the existing bridge and will have
one eastbound lane and one westbound lane with two left turn lanes. The new bridge will be offset to
the north of the existing bridge far enough that it can be built in its entirety while the current bridge
remains open. The grade of the new bridge will be raised 6 feet higher than the current bridge to
accommodate the 17 feet of vertical clearance mandated under the new interstate standards. The
bridge design includes sidewalks on both sides. The northbound entrance ramp to US 220 will have two
lanes to receive traffic from the two left turn lanes, which will merge into a single lane before merging
onto US 220. Entry to Lowes and Little Caesar's will be changed to right in/right out only access.
The east and west Y-line segments of NC 135 are proposed to be three 12- to 14-foot-wide lanes with a
curb and gutter drainage system. All of the discharge points of the curb and gutter system are located
on grassed slopes with rip rap flow diffusers. The four ramps are proposed to be 12- to 14-foot-wide
lanes with open vegetated shoulders that drain to grass lined roadside ditches. Stormwater will be
conveyed through grass lined roadside ditches, which drain to project streams.
During construction, silt fences, rock check dams, matting, and other erosion controls will be installed
per the guidance provided in the NCDOT Erosion and Sediment Control Design and Construction Manual
(2015) to treat runoff and prevent sediment laden discharge from leaving the work area.
Typical construction equipment and machinery will be used for grading, excavation, and construction
including track hoes, bulldozers, loaders, trenchers, and trucks.

D. Proposed Impacts Inventory
Proposed Impacts to jurisdictional waters:
S1 and S2 (Sheets 2 and 3):
S1 and S2 are located adjacent to the entry road to Dalton McMichael High School. The proposed work
will expand the roadway slope and extend the existing culvert, which will impact the perennial
jurisdictional stream (Stream SC Reach 3) at that location. At S1, approximately 30 linear feet of the
channel will be temporarily impacted by construction activities. At S2, approximately 16 linear feet of
the channel will be permanently placed in a 54-inch reinforced concrete pipe (RCP) culvert extension.
Areas temporarily impacted will be returned to pre -construction grade when work is complete.
W1 and W2 (Sheets 4 and 5):
W1 and W2 are located on the east side of ramp D near the north end of the merge lane. The proposed
work will require reshaping the swale at the toe of the roadway fill slope to facilitate stormwater
drainage. This will require both excavation and fill. At W1, less than 0.01 acres of permanent impacts to
a jurisdictional wetland (Wetland WA) located within the drainage swale are proposed. At W2, less than
0.01 acres of the wetland will be temporarily impacted by construction activities. Areas temporarily
impacted will be returned to pre -construction grade when work is complete.

S3 (Sheets 4 and 5):
S3 is located on the east side of ramp D, south of S2, near the north side NC 135 Y-line entry to the
ramp. The drainage swale will be routed through a RCP conveyance. The intermittent jurisdictional
stream (Stream SD Reach 2) currently located in the swale will also be included in that pipe, which will
result in 211 linear feet of permanent impact.
S4 (Sheets 4 and 5):
S4 is located on the east side of ramp C, south of NC 135. The intermittent stream impacted at S3
(Stream SD Reach 2) currently enters an RCP culvert section to pass under NC 135 and outlets to the
south of the interchange. The proposed work at S4 will permanently impact 7 linear feet of the stream
channel (Stream SD Reach 1) by removing and replacing the existing culvert and installing rip rap outlet
protection. The culvert will not be extended.
S5 (Sheets 4 and 5):
S5 is the reach of Stream SD Reach 1 parallel to ramp C will be temporarily impacted by construction
activities, totaling 925 linear feet of the reach within the limits of construction. Areas temporarily
impacted will be returned to pre -construction grade when work is complete.
S6 (Sheets 9 and 10):
S6 is located on the west side of ramp B. A perennial stream (Stream SC Reach 2) currently passes under
the interchange through an RCP west to east. The proposed work at S6 will temporarily impact 41 linear
feet of the stream channel at the inlet within the limits of construction due to construction activities.
Areas temporarily impacted will be returned to pre -construction grade when work is complete.

project:
Impacts to jurisdictional waters have been minimized by utilizing ditches, cross pipes, junction boxes,
and rip rap outlet pads designed and sized to minimize erosive discharge velocity. Silt fence and other
erosion controls will be installed per the guidance provided in the NCDOT Erosion and Sediment Control
Design and Construction Manual (2015) to prevent sediment laden discharge from leaving the work
area. Pipe networks were utilized to preserve stream flow in areas where existing jurisdictional streams
are impacted by proposed roadway design and fill. To reduce permanent fill encroaching into existing
jurisdictional streams 2:1 roadway slopes are used.

NCDEQ-DMS will commit to implement sufficient compensatory mitigation credits to offset the impacts associated
with this project as determined by the regulatory agencies using the delivery timeline listed in Section F.3.c.iii of the In -
Lieu Fee Instrument dated July 28, 2010. If the above referenced impact amounts are revised, then this mitigation
acceptance letter will no longer be valid and a new mitigation acceptance letter will be required from NCDEQ-DMS.

The North Carolina Department of Transportation (NCDOT) has begun planning and environmental
studies for the proposed transportation improvements to US 311/NC 135 Interchange, Rockingham
County. The project proposes to widen Bridge No. 780074 and Upgrade Interchange in TIP Project
1-5898.
As part of the preparation required for building this project, engineers, surveyors, geologists, and
others representing NCDOT will be working on or near your property for the next several months
collecting data necessary for the design of this transportation project. Other State and Federal
Agency representatives relevant to the project may also be present with NCDOT to ensure that the
data collected conforms to matters within their respective jurisdictions. The specific area in which
these representatives will be working does not necessarily indicate the final location or extent of the
proposed transportation improvement.
These representatives will be wearing high visibility safety vests, and will be locating and measuring
utilities, property corners, transportation features, and the limits of streams and wetlands. These
representatives will use wooden stakes, various colors of flagging, or ribbons, hung on trees and
shrubs as part of their work. These stakes and flagging do not indicate the location of the proposed
transportation project, but they are very important in our planning and environmental project
development process. Please do not disturb these stakes and flagging.
As the study progresses, newsletters will be mailed and/or advertised public meetings may be held
to present information and any preliminary designs to keep you informed of the project's progress.
