Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency dated October 15, 2007 (or as subsequently amended).
This permit contains no changes from the Draft permit mailed to you on March 25, 2009.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you
have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This
request must be in the form of a written petition, conforming to Chapter 150E of the North Carolina General Statutes, and
filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require modification
or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits
which may be required by the Division of Water Quality or permits required by the Division of Land Resources, the
Coastal Area Management Act or any other Federal or Local governmental permit that may be required. If you have any

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge
are hereby revoked. As of this permit issuance, any previously issued permit bearing
this number is no longer effective. Therefore; the exclusive authority to operate and
discharge from this facility arises under the permit conditions, requirements, terms,
and provisions included herein.
The City of Thomasville is hereby authorized to:
1. Continue to operate a Conventional Water Treatment Plant (WTP) producing up to
0.30 MGD of discharged clarified supernatant with the following
components/processes:

2. The facility is at the City of Thomasville WTP located on Old Lexington Road, near
Thomasville, in Davidson County.
3. Monitor and report results from three groundwater monitoring wells: MW-1, MW-2
and MW-3.
4. Discharge of filter -backwash wastewater from said treatment works at the location
specified on the attached map to an unnamed tributary to Rich Fork Creek,
classified C waters in the Yadkin Pee Dee River Basin.

1. For instantaneous flow monitoring the duration of the discharge must bel eported.
2. The Division shall consider all effluent TRC values reported below 50 ug/I: to be in compliance with the
permit. However, the Permittee shall continue to record and submit all values reported by a North Carolina
certified laboratory (including field certified), even if these values fall below 50 ug/I.
3. Facilities using chloramination will be required to monitor ammonia nitrogen on a quarterly basis.
4. Parameters should be monitored in conjunction with toxicity test.
5. Zinc should be monitored if facility uses zinc orthophosphate as a corrosion inhibitor.
6. Fluoride monitoring applies if the facility backwashes with fluoridated, finished water.
7. See Special Condition A. (2.).
All samples collected should be from a representative discharge event.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

A (2.) CHRONIC TOXICITY MONITORING (QRTRLY)
The permittee shall conduct Quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The testing
shall be performed as a Ceriodaphnia dubia 7 day pass/fail test. The tests will be performed during
the months of January, April, July and October. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter code
TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later •
than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for. :
disinfection of the waste stream. ,.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
,required,the permittee will complete the information located at the top of the aquatic toxicity (AT) test •
form indidating the facility name, permit number, pipe number, county, and the month/year of the report
with the notation of "No Flow" in the comment area of the form. The report shall be submitted to the
Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to' be completed no later than
the last day of the month following the month of the initial monitoring.

A. (3). GROUNDWATER MONITORING REQUIREMENTS
During the period beginning upon the effective date of the permit and lasting until expiration, the Permittee is
required to monitor the three existing monitoring wells: MW-1, MW-2, MW-3. Sampling shall be conducted each
November for the following parameters:

Water levels shall be measured prior to sampling for the remaining
parameters. The depth to water in each well shall be measured from the
surveyed point on top of each casing. The point atop the casing shall be
surveyed relative to a common datum.

Laboratory must be Division -certified for this parameter.
Chromium
Laboratory must be Division -certified for this parameter.
Lead
Laboratory must be Division -certified for this parameter.
Nickel
Laboratory must be Division -certified for this parameter.
pH
Laboratory must be Division -certified for this parameter.
Total Dissolved Solids (TDS)
Laboratory must be Division -certified for this parameter.
Total Organic Carbon (TOC)
Laboratory must be Division -certified for this parameter. If concentrations
greater than 10 mg/L are detected in any down -gradient monitoring well,
additional sampling & analysis must be conducted to identify the individual
constituents comprising the TOC concentration.
If the concentration as measured in the background monitoring well exceeds
10 mg/L, this concentration will be considered representative of the naturally
occurring TOC concentration. Any exceedence of this naturally occurring
concentration in the down -gradient wells shall be subject to the additional
sampling & analysis as described above.

Laboratory must be Division -certified for this parameter. Laboratory must use
._ . only one of the following methods:
1. Standard Method 6230D, Practical Quantitation Limit (PQL) at 0.5 pg/L
:. or less;
2. Standard Method 6210, PQL at 0.5 pg/L or less;
3. EPA Method 8021., Low Concentration, PQL at 0.5 pg/L or less;
4. EPA Method 8260, Low Concentration, PQL at 0.5 pg/L or less;
. 5. OR another method with prior approval from the Aquifer Protection
Section Chief.
Any of the referenced methods must [at a minimum] include all the
constituents listed in Table VIII of Standard Method 6230D. Any method
used must have a PQL at or below 0.5 pg/L, supported by laboratory
proficiency studies as required by the Division's Laboratory Certification Unit.
Any constituents detected above the Method Detection Limit (MDL) but below
the PQL must be qualified [estimated] and reported.
Reoortinct / Documentation Requirements
a. Sampling & analytical results shall be submitted to the Division's Aquifer Protection Section on or before
the last working day following the sampling month. Data shall be reported on the most recent version of
the "Groundwater Quality Monitoring: Compliance Report Form" [i.e. the GW-59 Form] with copies of the
laboratory analyses attached.
b. All reports, maps, and other documents shall be mailed to:

Applicable Boundaries
a. The COMPLIANCE BOUNDARY for the surface disposal unit is specified by 15A NCAC 2L
[Groundwater Classifications and Standards]. The Compliance Boundary for the disposal system
individually permitted on or after December 31, 1983 is established at either 250 feet from the waste
disposal area or 50 feet within the property boundary, whichever is closer to the waste disposal area.
An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to
remediation action according to 15A NCAC 2L.0106 (d)(2).
b. The REVIEW BOUNDARY is established around the surface disposal unit midway between the
Compliance Boundary and the perimeter of the surface disposal unit. Any exceedance of Groundwater
Quality Standards at the Review Boundary shall require action according to 15A NCAC 2L.0106 (d)(1).
Additional Requirements
Any additional groundwater quality monitoring deemed necessary by the Division shall be performed by the
Permittee.

discharge from this facility has ceased at this time, the City does not wish to rescind this permit.

Potassium Permananate is added for Iron and Manganese control at Raw Water Pump
Station. All other chemicals are added at the Water Treatment Plant. Alum is added for
coagulation. Caustic is added for pH control. Chlorine is added to kill bacteria. Fluoride
for healthy teeth. Polyphosphate for corrosion control. Sodium Bisulfate added to the
alum sludge for dechlornation.

facility:
350-gpm pump station, force main (ca. 14,000 linear feet of 8" pipe), residual receiving
station, 2.1-acre residual settling lagoon, decanting structure, flow meter, rip -rap effluent
discharge channel, three groundwater monitoring wells, and dechlorinating feed system.
Note that Alum Residual Lagoon has received no wastewater since late February 2008,
pending cleanout of sludge in the lagoon. Alum waste is currently discharged to the City
sanitary sewer for treatment at the WWTP.

North Carolina General Statute 143-215.6 (b)(2) provides that: Any person who knowingly makes any false
statement representation, or certification in any application, record, report, plan, or other document files or
required to be maintained under Article 21 or regulations of the Environmental Management Commission
implementing that Article, or who falsifies, tampers with, or knowingly renders inaccurate any recording or
monitoring device or method required to be operated or maintained under Article 21 or regulations of the
Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable

Notes on Preparation of Permit Renewal for NC0088200
No effluent has been discharged from the lagoon since the middle of March 2008. No waste has been
sent to the lagoon since late February 2008. Currently the water plant alum waste is transported through
the sanitary sewer to the Thomasville WWTP.
Sludge Management Plan
This facility does not currently have a sludge management plan, however several companies have been
approached to provide solutions for long term sludge management at this facility. Some options include
using the material as an amendment in the manufacture of bricks, or as an amendment to composting
materials (if it can be verified that this meets the 503 regulations). Additional funding has been requested
for further study of the options. The City is still evaluating options for resolution to the sludge removal
and disposal issue. A sludge management plan will be developed once all options have been evaluated
and before the lagoon is to be put back in service. In the mean time, sludge generated by the City's Water
Plant from alum treatment of the water, will be treated and disposed of through the City's Wastewater
Plant. Sludge disposal through the WWTP is handled by landfill of sludge at a Subtitle D lined landfill or
through land application under 503 regulations.
