Inspection Summary:
On June 14, 2022, staff of the North Carolina Division of Water Resources Winston-Salem Regional Office (DWR) performed
a reconaissance inspection of the subject facility in conjunction with a UIC permit modification application review. This
inspection was conducted by DWR staff person Jim Gonsiewski. The inspection concentrated in the areas that will be part
of the planned expansion of the injection system. The facility is very well maintained. No problems with the proposed
modification were noted.
