Inspection Summary:
On June 2, 2022, Tricia Lowery with the Division of Water Resources conducted a very brief compliance evaluation
inspection of the Mill Ridge Collection System (WQCSD0234). Mr. Orlowski, resident manager, was unaware an inspection
was scheduled. Paul Isenhour with Water Quality Labs forwarded an email to inspector post inspection that was sent to Mill
Ridge prior, informing them of upcoming inspection. Inspector cannot confirm if Mr. Orlowski was alerted or forwarded the
email, as it is a general inbox for Mill Ridge property. Results of the inspection are summarized in the following paragraphs:
Mill Ridge is a small community, primarily seasonal with no pump stations and one high priority line (aerial) that is located
over a creek and under a walking bridge. The inspection was very brief as Mr. Orlowski informed inspector he was currently
working on an active water leak in community.
15A NCAC 02T .0403 Permitting By Regulation
(a)(1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface
waters and to prevent any contravention of groundwater standards or surface water standards.
Observations: Due to circumstances mentioned previously, a thorough inspection could not be conducted, however,
inspector was able to determine there are some areas of concern and will be further discussed in report.

(a)(2) A map of the collection system shall have been developed and shall be maintained.
Observations: Map of system is located on Mr. Orlowski's phone and inspector was not able to see a physical print of map.
Appeared to be a basic map. Inspector did not received a copy of map via email to further investigate to see if all required
parameters were embedded into map.

(a)(3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule,
spare parts inventory, and overflow response shall have been developed and implemented.
Observations: There is only one log Mr. Orlowski uses as a daily operational log. No documented spill repsonse plan,
maintenance schedule, or spare parts inventory.

(a)(4) Pump stations that are not connected to telemetry system shall be inspected by the permittee or its representative
every day, 365 days per year, unless the permittee demonstrates that the daily inspections are not necessary because the
pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection
interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry
system shall be inspected once per week.
Observations: No pump stations in collection system

(a)(5) High -priority sewers shall be inspected by the permittee or its representative once every six months, and inspections
shall be documented.
Observations: Only one HPL in collection system. Mr. Orlowski stated he observed HPL daily. No documentation given to
inspector to confirm this.

(a)(6) A general observation by the permittee or its representative of the entire collection system shall be conducted once
per year.
Observations: Mr. Orlowski stated he oversees collection system daily. No documentation was given to inpsector to
confirm.

(a)(7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC
02B .0506(a), and public notice shall be provided as required by G.S. 143-215.1 C.
Observations: No SSO or bypasses reported to Division and Mr. Orlowski stated the collection system has no bypasses or
overflows to report.

(a)(8)(B) Grease Control Program shall be in place for private owned collection systems that will include bi-annual
distribution of grease education materials to users of the collection system by the permittee or representative.
Observations: Mr. Orlowski stated the collection system does not have a grease program and no distribution concerning

FOG education is distributed.

(a)(9) Right-of-ways and easements shall be maintained in the full easement width for personnel and equipment
accessibility.
Observations: Inspector was unable to observe right of ways due to circumstances mentioned at beginning of summary.

(a)(10) Documentation of compliance with Subparagraphs (a)(1) through (a)(9) of this Rule shall be maintained by the
collection system owner for three years with the exception of the map, which shall be maintained for the life of the system.
Observations: Documentation is limited in collection system. There is only one generalized log of operations and
maintenance of collection system.

Comment: Mr. Orlowski had an overall daily inspection log for inspector to review. This is the only log for
collection system and similar to a wastewater operator's daily log. There are no pump stations
in this system. The system has one HPL located under a bridge that crosses over creek.

Comment: No dedicated log for aerial, right of ways. All information concerning inspections, maintenance,
