Statesville Ave, Mooresville, NC 28115 (704)662-8192 Fax- (704)662-8194 April 15, 2022 We have completed the stormwater monitoring for the first quarter 2022, and the Stormwater Discharge Outfall (SDO) Monitoring Report package to satisfy the sampling requirements of Permit NCG030478. Please find enclosed the following items: 1. Suggested cover letter to the NCDEQ Fayetteville Regional Office We have forwarded a copy in Word via email to print. Ifyou have not already mailed the signed original DMR submission to the FRO with the Word copy of the cover letter we emailed you, please sign the enclosed copy and submit with a signed original of the DMR 2. Stormwater Discharge Outfall Monitoring Report (DMR) A responsible corporate officer or their designee must sign the DMR. We will upload the signed DMR to NCDEQ forward you the documentation. 3. Stormwater Discharge Outfall Qualitative Monitoring Reports (QMR) A responsible corporate officer or their designee must also sign the QMRs. The signatory should be the same person that signs the DMR. The enclosed QMR for each outfall must be signed and kept on file with the stormwater pollution prevention plan. 4. Field analytical results One copy of the field analytical pH results report is included to be kept on f le. 5. Laboratory analytical results One copy of the laboratory analytical results report is included to be kept on file. 6. Stormwater Sampling Documentation Summary One copy of the stormwater sampling documentation summary is included to be kept on f le. Please sign the suggested cover letter and submit with one original signed DMR report to the following address: DEQ Fayetteville Regional Office Attn: DEMLR Stormwater Program 225 Green Street, Suite 714 Fayetteville, NC 28301-5095 Signed copies of the DMR and QMR reports, field and laboratory results, and the other sampling documentation must be kept on file at the permitted facility site location for at least 5 years from the sampling date. The next quarterly sampling event must occur before .Tune 30, 2022, as outlined in the new permit.
