Dear Mr. Stewart,
On May 15, 2014, the Division of Water Resources (Division) received your request for a
modification to an Individual Water Quality Certification (3787) from the Division for the subject
project. The Division has determined that your application is incomplete and cannot be processed.
The application is on -hold until all of the following information is received:
A review of the information submitted indicates this application is for an additional 20
linear feet of stream impacts. Pursuant to 143- 2145.3D (e), a $240 permitting fee is
required to process this application. Please remit a check to DENR -DWR, 1650 Mail Service
Center, Raleigh, NC 27699 -1650.
2. The Division requires four (4) complete application packages, including the appropriate fee,
to begin the proper consideration of an application (pursuant to 15A NCAC 02H 0502 and
G.S. 143 - 215.3). For more information about the approval process and fees, please visit•

additional three (3) copies of this modification request
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested
information for the proper consideration of the application.
If all of the requested information is not received in writing at the address below within 30
calendar days of receipt of this letter, the Division will be unable to process the application and it
will be returned. The return of this project will necessitate reapplication to the Division for
approval, including a complete application package and the appropriate fee.
