This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of , Agreement between North Carolina and the US Environmental Protection Agency dated October -15, 2007 (or as subsequently amended). Please take notice that this certificate of coverage is not transferable except after notice to the Division of Energy, Mineral, and Land Resources. The Division may require modification or revocation and reissuance of the certificate of coverage (COC). -.r Coverage under this General Permit is granted with the condition_ that the permittee performs additional sampling`requirements as outlined in the amended-co, mended conditions to the COC. These include: _ (1) In addition to the parameters in NCG120000, sample each SDO semi-annually for conductivity, pH, arsenic (As), selenium (Se),and boron (B). Thacker January 6, 2014 Page 2 of " These additional parameters include metals and other constituents which may be present in Zero Liquid Discharge (ZLD) wastewater used to condition the coal combustion residue (CCR) transported to the monofill. These sample results will provide a baseline for stormwater discharges when the monofill has just begun operation and a comparison after three years of operation. The Division will consider the data upon renewal of coverage under this General Permit. The facility may request `Representative Outfall Status" for one or more outfalls at the mono -fill so flhat fewer outff4f,; must be sampled. Please see Footnote 3 to Table 1 in the amended COC. A form for an ROS Request can be found on our website at http://12ortal.ncdenr.org-lweb/Jr/npdes-stormwater (on the "Miscellaneous Forms" tab). We request that you provide copies of in -stream monitoring results per the approved Water Quality Monitoring Plan (WQMP) required by the Solid Waste Permit for this monofill site to the Raleigh Regional Office. Copies of these results should be sent to the attention of Dave Parnell in the NC DEMLR Stormwater Permitting Program. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Energy, Mining, and Land Resources, or permits required by the Division of Water Resources, Coastal Area Management Act, or any other federal or local governmental permit that may be required. If the conditions as stipulated in the COC are unacceptable to you, you may apply for an individual NPDES stormwater permit.
