Box 968 Asheboro, NC 27204 Subject: Request for Additional Information Stormwater Permit No. SW5190901 Deliverance Church Chatham County Dear Mr. Summey: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on September 26, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. The name of the signing official was not printed in the Applicant's Certification (Part X of the application). Please resubmit the application with the certification completed. 2. The registered agent listed on the NC Secretary of State's website did not sign the application. The application must be signed by an executive officer or an authorized representative [15A NCAC 02H .1040(1)(a)]. Alternatively, a letter from the registered agent or other official documentation granting authority to the individual who signed the application is acceptable. 3. The plans and report show that there are streams crossing the property. 15A NCAC 02H .1003(1)(c) requires the project area to exclude the surface area of any waterbodies. Please revise Part IV of the application to subtract the surface water area from the project area. 4. The plans and report state that the proposed built -upon area (BUA) is 20,011 sf and that the project density is 6.95%; however, the application states that the proposed BUA is 69,121 sf and the project density is 24%. Please correct the discrepancy. 5. Please provide a detail for the swales and ensure that both the detail and plans show the side slopes of the swales as no steeper than 3:1 [15A NCAC 02H . 1003 (2)(c)(i)]. Please submit a low density Supplement EZ form and include calculations showing that the vegetated conveyances will not have erosion during the peak flow from the 10-year storm [15A NCAC 02H .1003(2)(c)(ii)]. The Supplement EZ form can be found on the Stormwater Design Manual website. 7. Please revise the plans to show the swales in a permanent recorded easement [15A NCAC 02H .1050(9)]. 8. Please provide an Operation and Maintenance Plan and Agreement for the swales [15A NCAC 02H .1050(11) & 15A NCAC 02H .1050(12)]. You can use O&M EZ to meet this requirement. O&M EZ can be found on the Stormwater Design Manual website. 9. Please revise the grading plan to show the location of all roof downspouts. 10. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: hqps:Hedocs.deg.nc.gov/Forins/SW ProjectSubmittal Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to November 15, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW5190901 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
