This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in NPDES WW Permit No. NC0028223. The findings and comments noted during this inspection are provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report". There were no significant issues or findings noted during the inspection and therefore, a response to this inspection report is not required. The purpose of the inspection was to verify that the facility is operating in compliance with the conditions and limitations specified within Permit NC0028223. Jeffrey Jarman (ORC) was present during the inspection. The current permit was issued 6/21/2017 and was modified to provide for ownership change and reissued 2/7/2020 with an expiration date of 6/30/2022. The operator was reminded that permit renewal applications need to be at least 180 days prior to the expiration date. The system is permitted for a 40,000 gallon per day WWTP which generally includes: o An influent duplex pump station; o A basket screen on effluent line into primary aeration basin; o A below grade rectangular -50,000-gal primary aeration basin; o A below grade rectangular clarifier with v-notched weir; o A below grade circular -40,000-gal secondary aeration basin; o Two below grade circular clarifiers installed in parallel; o A below grade polishing pond (or lagoon as indicated in inspection report or Settling pond within the permit); o A liquid (12.5% solution of sodium hypochlorite) chlorination system, injected into effluent of polishing pond; o Calcium thiosulfate for de -chlorination; o Ferrous sulfate is injected into the primary aeration basin for phosphate removal; o An aerated sludge digester The system has had an average daily flow of approximately 19,000 gpd over the period of 1/1/2017 through 3/31/2022, although these flows have been increasing since 2019 with the average daily flow for 2021 being -25,000 gpd. A review of monitoring records indicates that since the current permit became effective in 6/2017, the system has had the following violations: - Exceedances of daily BOD5 limits in the effluent based on sample results obtained on 3/21/2018 (also for monthly average 3/2018) and 8/31/2017 (also for 8/2017 monthly average), 11/13/2018, 3/21/2018 (also monthly average for 3/2018), 9/20/2017, 9/27/2017 (also for 9/2017 monthly average) and 6/7/2017 - Exceedances of daily enterococci limits in the effluent based on samples obtained on 5/27/2020 - Failing to achieve the minimum dissolved oxygen of 6.0 mg/L in the effluent on 5/29/2019, 5/15/2019 and 5/8/2019 The operators were very knowledgeable of the facility and the facility seemed to be well maintained. The facility is generally compliant with the discharge Permit NC0028223. Please review the comment sections of the inspection report for more details. The modified permit expires 6/30/2022 and the operator was informed that a renewal should be submitted 180 days prior to the expiration date. The operator indicated that he is working on the submittal of a permit renewal application. Although a wooden fence has been installed surrounding the plant, the fence is in need of repairs or replacement. In 2020 a man was found deceased in the polishing pond, albeit when construction activities were ongoing and fencing was partially removed. Considering this information and the design of the system, secure fencing is imperative for proper security. Compliance Schedules Is there a compliance schedule for this facility? The old wood building and wet well have been removed and a prefabircated concrete wet well with concrete top and controls mounted on plywood panel are present. In addition, another system called Southgate MHP WWTF (NC0051853) discharges into the same unnamed tributary. The outfall for Southgate is approximately 990' downqradient of the chlorine contact chamber or -750 feet south of Outfall 001 for Osprey Cove So. Since the outfall for Southgate is within 0.2 miles of the permitted downqradient sample point, system staff have been obtaining samples upqradient of the Southgate outfall. It is recommended that the permit renewal downqradient sample point for Osprey Cove South be changed to reflect a downgradient sample point less than 0.2 miles from Outfall 001 and upqradient of the Southgate MHP WWTF outfall.
