and turn right onto W. Bladen Street. Follow W. Bladen St. until it terminates at the Bladenboro WWTP. The land
application fields are on both sides of W. Bladen St.

III. EXISTING FACILITIES: MODIFICATION AND RENEWAL APPLICATIONS

Description of existing facilities: 0.500 MGD wastewater plant that primarily treats domestic wastewater with
minimal industry and/or typical commercial flow (fast food, laundry, etc.).
Proposed flow: 0.500 MGD (NPDES)
Current permitted flow: 0.500 MGD (with current average flow of 0.125 MGD).
Explain anything observed during the site visit that needs to be addressed by the permit, or that may be important
for the permit writer to know (i.e., equipment condition, function, maintenance, a change in facility ownership,
etc.) N/A
3. Are the site conditions (e.g., soils, topography, depth to water table, etc.) maintained appropriately and adequately
assimilating the waste? ® Yes or ❑ No
If no, please explain. All fields used in the Bladenboro residuals land application program are adjacent to the
wastewater treatment plant. A local farmer manages the fields for the town and has all fields in a corn, soy_bean
wheat rotation.
4. Has the site changed in any way that may affect the permit (e.g., drainage added, new wells inside the compliance
boundary, new development, etc.)? ❑ Yes or ® No
If yes, please explain: The current buffer maps prepared by yLiagro during the last permit renewal are acceptable
and represent the area around the wastewater plant and farm fields.

If no, please explain: The Bladenboro WWTP is permitted for 0.500 MGD, but currently ages approximately
0.125 MGD and therefore, residual generation is well below the permitted amount.

If no, explain and recommend any changes to the groundwater monitoring program: Groundwater monitoring
the Bladenboro land application fields is related to the conjunctive spray irrigation permit issued to Bladenboro
for managing a portion of their wastewater.

Please summarize any findings resulting from this review: Annual report reviewed for 2021, showing 12.42 dry
tons of residuals were distributed. Nothing for 2020. In 2019, 27.96 dry tons of residuals were distributed.
Nothing for 2018. No adverse issues noted.
