Director NORTH CAROLINA Environmental Quality June 15, 2022 Mr. Raymond Gottlieb — Managing Partner Sandler Utilities at Mill Run, L.L.C. 448 Viking Drive — Suite 220 Virginia Beach, VA 23452 SUBJECT: 06/10/2022 Compliance Inspection Eagle Creek WWTP Permit No: WQ0014306 (Expires 06/30/2027) Currituck County Dear Mr. Gottlieb: Attached to this letter please find a copy of the Compliance Inspection Report Form for the Compliance Inspection conducted on June 10, 2022 at the above referenced facility. The Compliance Inspection was conducted by Randy Sipe and David May of the Washington Regional Office. Mike Myers and Tracy Miller with Envirolink, the company operating the facility, were also present during the inspection. During the inspection the wastewater treatment, storage, and disposal components of the facility described in Permit No. WQ0014306 were toured. The inspection also included a review of monthly reports/recordkeeping. At the time of the inspection, the operation of the facility was found to be in general compliance with permit No. WQ0014306. However, the following outstanding compliance issues, related to conditions in Permit No. WQ0014306, were identified during the inspection. • A new site map has not been provided to the Division, as required by Condition I.4 of the permit • The staff gauge at the infiltration basin has been repaired since the last inspection, but still needs to be calibrated to the top of the berm around the basin in order to comply with Condition I.7 of the permit. • The woody vegetation around the infiltration basin and the reclaimed storage pond has not been removed, as required by Condition I.10 of the permit. • A plan to clean out the infiltration basin has not been submitted to the Washington Regional Office, as required by Condition I.11 of the permit Please be aware that if any of these issues identified during the inspection continue, it may result in issuance of a Notice of Violation (NOV) or initiation of other enforcement mechanisms available to the state.
