received March 28, 2022. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on July 14, 2022.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items

1. The City of Jacksonville WWTF was permitted for expansion from 6.0 million gallons per day
(MGD) to 9.0 MGD on July 9, 2008 which constitutes a major modification. Pursuant to 15A
NCAC 02T .0105(d), please submit a site map pursuant to the requirements in 15A NCAC 02T
.0504(d). These requirements are:
a. A scaled map of the site with topographic contour intervals not exceeding 10 feet or
25 percent of total site relief and showing all facility-related structures and fences
within the wastewater treatment, storage, and irrigation areas.
b. Soil mapping units shown on all irrigation sites.
c. The location of all wells (including usage and construction details if available), streams
(ephemeral, intermittent, and perennial), springs, lakes, ponds, and other surface
drainage features within 500 feet of all wastewater treatment, storage, and irrigation
sites.
d. Delineation of the compliance and review boundaries per 15A NCAC 02L .0107 and
.0108, and 15A NCAC 02T .0506(c) if applicable.
e. Setbacks as required by 15A NCAC 02T .0506.
f. Site property boundaries within 500 feet of all wastewater treatment, storage, and
irrigation sites.
g. All habitable residences or places of public assembly within 500 feet of all treatment,
storage, and irrigation sites.

 Existing Permit:
1. The facility description, from the current permit version dated November 9, 2017, lists several
modifications to the mixers/aerators, and it is unclear what the horsepower ratings are for the
installed Blue Frog and Gold Frog mixers/aerators. Please provide a table that specifies the
location, brand/type, horsepower, and number of units for the mixers/aerators installed at each
location.
