Dear Mr. Kegley:
On behalf of our client (G. Tyson Hopkins Associates, LLC (GTH)), Highlands Environmental
Solutions, Inc. (HES) is submitting this application to install one off -site groundwater monitoring
well as a condition of the completion of a Comprehensive Site Assessment (CSA) per the Notice
of Regulatory Requirement (NORR) letter dated May 6, 2022. A copy of the letter is included in
Attachment A.
Based on the findings during the Phase I Limited Site Assessment (LSA) completed by HES,
NCDEQ has required GTH to perform an CSA on the property. A total of four monitoring wells
were constructed during LSA site activities. Since the installation of the initial wells did not define
the lateral and vertical extent of the groundwater contamination, HES is proposing the installation
of additional wells to include one off -site monitoring well during CSA project activities. The
location of the GTH site is depicted on Figure 1, included in Attachment B.
The scope of work for the CSA includes the installation of additional groundwater monitoring wells
with one to be constructed on the subject site, and the second on the adjacent property located
Northeast and downgradient of the area of petroleum -impacted soil. The locations of property
boundaries and proposed well locations are depicted on Figure 2. Each well will be an EPA
Type II groundwater monitoring well. The off -site well will be installed to a maximum anticipated
depth of 12 feet below grade. The well will be constructed of two-inch Schedule 40 PVC
materials, and completed with a flush mount metal cover and locking expansion cap. A
schematic of the proposed well is included as Figure 3.
Because one of the wells will be installed on a third -party property, we are required to obtain a
Monitoring Well Permit from the NCDEQ. The completed Monitoring Well Permit Application is
included in Attachment C.

Information received by the UST Section, Division of Waste Management,Wilmington Regional
Office in the Limited Site Assessment on April 8, 2022, has been reviewed. Information in the report
confirms a release or discharge of petroleum product from an aboveground storage tank system as the
result of a spill at the above-referenced location. This office has determined that you are responsible for
the release or discharge.
As a responsible party, you must comply with assessment and cleanup requirements of TitlelSA
NCAC 2L .0106(c)(3) and 2L .0106(g). A Comprehensive Site Assessment (CSA) Report prepared in
accordance with these requirements and the most recent version of the UST Section Guidelines for the
Investigation and Remediation of Contamination from Non-UST Petroleum Releases must be received by
this office within 90 days of the date of this notice. In addition, you must submit a summary of the CSA
Report to the local Health Director and the local Chief Administrative Officer in accordance with 15A NCAC
2L .0114. The summary should be submitted to these persons no later than five working days after
submittal of the CSA Report to this office. Failure to comply in the manner and time specified,may result
in the assessment of civil penalties and/or the use of other enforcement mechanisms.
Because a release or discharge has been confirmed,a Licensed Geologist or a Professional Engineer,
certified by the State of North Carolina, is required to prepare and certify ail reports submitted to the
Department of Environmental Quality in accordance with Title 15A NCAC 2L.0103(e)and 2L.0111(b).
