Subject: REQUEST FOR ADDITIONAL INFORMATION
Bessemer City Sewer System Improvements Project
Dear Mr. Inman,
On June 2, 2014, the Division of Water Resources (Division) received an'application requesting a
401 Water Quality Certification from the Division for the subject project. The Division has
determined that the application- is incomplete and cannot be processed at this-ti me. The
application is on -hold until the following items are addressed:
1. The Division requires five (5) complete application packages to begin the proper
consideration of an application (pursuant to 15A NCAC 02H .0502 and G.S. 143 - 215.3). For
more information about the`approval process and fees, please visit:

additional four (4) copies of this signed application including the small vicinity maps and
agent authorization form.. Do not need any more copies of the plans however.
2. Please send the information requested above within 30 calendar days of your receipt of
this letter to Shelton Sullivan:

This letter only addresses the initial administrative review and does not authorize any impacts to
wetlands, waters or protected buffers. Pursuant to Title 15A NCAC 02H .0502(e), the applicant
shall furnish all of the above requested information for the proper consideration of the
application. If all of the requested information is not received in writing within 30 calendar days of
your receipt of-this letter, the Division will be unable to process the application and it will be
returned. The return of this project will necessitate reapplication to the Division for approval,
including a "complete application package and the appropriate fee. Please be aware that there is
no authorization under the Section 401 of the Clean Water Act for this activity and any work done,
within - waters of the state may be a violation of North Carolina General Statutes and
Administrative Code.
