On June 2, 2014, the Division of Water Resources (Division) received your application requesting a
401 Water Quality Certification from the Division for the subject project. 'The Division has
determined that your application is incomplete and cannot be processed. The application is on-
hold until all of the following information is received:
1. There was no Nationwide Permit Number or General Permit Number listed on the
application. Please contact Please contact Tasha McCormick
(Tasha.L.McCormick @usace.army.mil) with the U.S. Army Corps of Engineers to determine
what permit number this activity will be covered under.
2. The Division requires five (5) complete application packages, including the appropriate fee,
to begin the proper consideration of an application (pursuant to 15A NCAC 02H .0502 and
G.S. 143 - 215.3). We received five copies of the site plans, however we only received one
copy of the PCN with additional information. For more information about the approval

Pursuant to Title 15A NCAC 02H .0502(e) the applicant shall furnish all of the above requested
information for the proper consideration of the application.
If all of the requested information is not received in writing at the address below within 30
calendar days of receipt of this letter, the Division will be unable to process the application and it
will be returned. The return of this project will necessitate reapplication to the Division for
approval, including a complete application package and the appropriate fee.
