request has not been submitted by the permittee. We cannot process the NOT and end the permit coverage/annual
billing without the permittee taking this action. These instructions are also on the bottom of this Certificate of
Completion from the County.

Aziza has been extraordinarily busy with the volume of billing questions this month, and she has been doing her best to
get back to calls and emails, but it takes a lot of time. We have actually been without construction stormwater billing
support for almost a year, and she has been doing this in addition to her other designated duties! She is really
exceptional at customer service and spends a lot of her day helping many permittees who ask for guidance. We
appreciate your understanding. I will try to help you out today!
The close-out you need is from Chatham County. I have attached the original E&SC Plan approval documentation for
your reference, and I've copied someone with the County who can probably help further. We need close-out
documentation from the County when you fill out that Notice of Termination (NOT) Request, and they should be able to
provide what you need. If you ever need to check what the original E&SC documentation was for the original permit,
which will help you figure out where the close out documentation should come from, please check our Construction COC
Repository website for all the records. The folders are organized by permit number.

This is the second house that has closed and we don't have the correct information needed to complete your NOT form,
nor do we know where to find it.
This is my third request for more information so I can complete the NOT. I also left a VM with Aziza at least a month ago
without a reply. No one was in the office today when I tried Aziza again.
