You have requested a schedule for making partial payments toward the subjected civil penalties totaling
$12,341.22. The Division is agreeable to a monthly payment schedule beginning on June 1, 2022 and
continuing to November 1, 2023. There is Seventeen (17) payment of Six Hundred Eighty -Five Dollars
($685.00) to be made once every month and One payment of Six Hundred Ninety -Six Dollars and
Twenty -Two cents ($696.22) as indicated on the attached schedule.
Please sign the attached payment schedule and send it back with your first payment. Make a copy of the
payment schedule for your records. Please make the payments by check payable to the Department of
Environmental Quality and mail them so they are received by the 1st of each month.
Failure to make the monthly payments in accordance with this schedule will cause the entire unpaid
balance of the civil penalty to be due and payable and subject to collection through a civil action in the
superior court.
