Project No.: COH-DS22.001 SEC Dear Mr. Martinez: This letter is to confirm that the erosion and sedimentation control plans and all other documentation have been received and approved by the City of Henderson, Engineering Department in accordance with the City Ordinance, Chapter 23 for the PETCO, LLC, Permit Number COH-DS 22.001 SEC for 2.28 Acres of land disturbing activity contingent upon receiving the COC — certificate of coverage. COC are typically issued 3 days after you have filed your e-NOI (electronic -Notice of Intent) at deq.nc.gov/NCGO 1. This letter of approval is valid for 24 months from the date on this letter and is applicable for grading operations and construction/maintenance of erosion and sedimentation control measures only. An extension can be granted if the City's Erosion & Sedimentation Control regulations and site conditions have not changed. A copy of this letter of approval shall be posted at the project site throughout construction. The owner and/or his representative is responsible for performing, logging and maintaining the self - inspection reports for this project. The reports are to be kept onsite or available for review during inspections by the City. Any modifications or additions to the plans will require a modified submission to the City prior to commencing that portion of the operation. Advanced planning with submission of the modified plans at least 30 days prior to the additional work needed should be accounted to reduce or eliminate any delays to the project. COH-DS22.001 SEC Page 2 The City reserves the right to perform periodic inspections to ensure that the plan is being followed and that the measures approved on the plans are adequate to control the site from off -site sedimentation and erosion. If the approved measures are deemed insufficient from keeping sediment from going off -site, the City reserves the right to require additional measures as necessary. Prior to staring the project, please contact the City of Henderson Engineering Inspector at (252) 432-1914 for setting up a pre -construction meeting.
