On April 12, 2022, Gary Hudson of this office met with you to perform a Compliance
Evaluation Inspection at the above referenced wastewater collection system. This type
of inspection consists of two basic parts: an in -office file review and an on -site inspection
of the treatment facility. The attached EPA inspection form details the areas that were
evaluated during this inspection.

*The system has 3 pump stations that are checked thoroughly by Mr. Cheek during his WWTP visits.
*AII maintenance activities are kept in the main WWTP logbook.
*There is one high priority line at the entrance to the WWTP and it is inspected during every visit
*An Operation and Maintenance Plan as well as a Spill Response Action Plan was presented for inspection. Mr. Cheek is in
the process of updating these plans.
