Director Division of Water Quality DWQ Project # OS-0327 Crranville County Page 1 of 2 Subject Property: Carolina Sunrock Butner Quarry Stream Crossing Cedar Creek [03-04-01, 27-12-1-1-(1), C NSW (nutrient sensitive)] REQUEST FOR MORE INFORMATION Dear Sir or Madam: On February 4, 2005, the Division of Water Quality (DWQ) received your application dated October 29, 2004 to impact 1.40 feet of stream and 0 square feet (ftz) of protected riparian buffers to construct the proposed road crossing. The DWQ has determined that your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetland, streams and/or buffers on the subject property. Therefore, unless we receive the additional information requested below, we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and 2B .0233(8) and will place this project on hold as incomplete until we receive this additional information. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. There was no application fee included with this application, you are requesting to use a NW 39 from the USCOE, you will need written concurrence from the DWQ for a WQC 3402. Please send a check for $200.00 so the application can be reviewed. 2. Please provide all stream impacts including all fill slopes, dissipaters, culverts and bank stabilization to scale on the site plan (1"= 50'). 3. Please indicate all buffer impacts to scale on the site plan (1' = 50'). 4. Please indicate the location of the protected buffers as overlays to scale on the site plan (1' = 50'). 5. Please indicate the diffuse flow provision measures on the site plan. 6. If we do not hear from you within three weeks, we will assume that you no longer want to pursue this project and we will consider the project as withdrawn. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
