Director Barry Stanley, Public Works Director Town of Four Oaks PO Box 610 Four Oaks, NC 27524 Dear Mr. Stanley: NORTH CAROLINA Environmental Quality May 31, 2022 Subject: Compliance Evaluation Inspection Town of Four Oaks Wastewater Collection System Permit Number WQCSD0013 Johnston County Mitch Hayes of the Raleigh Regional Office conducted the subject inspection on May 27,2022. The assistance and cooperation provided by you, Chris Barbour, and Rhonda Lee was greatly appreciated. Below is a list of findings developed from the inspection and review of the permit: 1. The subject collection system is classified as a CS-2 collection system with an effective date ofJuly 18, 2003. You are listed as the CS-2 Operator in Responsible Charge (ORC) and Chris Barbour as the CS-1 back up ORC. 2. All wastewater flow is conveyed and treated at Central Johnston County Wastewater Treatment Plant in Smithfield. 3. A complete map of the sewer system was available for review. A working map of the collection shows the direction of flow and contains all system requirements. 4. The Town of Four Oaks has an enforceable fats, oils, and grease program and a sewer use ordinance. Bi-annual distribution of educational materials for commercial and residential users is documented. There have been no enforcement actions taken for the year. 5. All required logbooks are being maintained; right of way maintenance, high priority line inspection, operation and maintenance plan, overflow response plan, pump station log books. 6. There are five pump stations in the collection system. All pump stations were inspected. All pump stations have restricted access, contact information signage, North Carolina Department of Environmental Quality Division of Water Resources Raleigh Regions! Mr. Barbour stated he would replace the float that afternoon. The auto -transfer switch for the generator at Austin Acres pump station was out of service. Mr. Barbour stated that parts for the switch gear will not be in until August 2022. All other pumps functioned when their respective floats were lifted up. All audible visual alarms operated when the high water float was lifted up. Telemetry phoned in a high water alarm. All on -site generators operated when power was turned off to the pump station.
