Dear Mr. Whitfield,

Section A.6 of your permit requires triannual groundwater monitoring at seven monitoring wells
in the months of March, July, and November. On May 27, 2022, staff of the Division of Water
Resources (DWR), Asheville Regional Office (ARO) determined that the groundwater monitoring
reports for November 2020 and July 2021 were not submitted to the Information Processing Unit
as instructed on the GW-59 reporting form.

DWR ARO is aware that groundwater monitoring was conducted on November 9, 2020 and July
7, 2021. You are required to resubmit the groundwater monitoring reports to the Information
Processing Unit within 30 business days of receipt of this letter. You will also be required to
provide notification to this office when these actions are completed.
