Dear Mr. Horton,
As you are aware, your non-discharge wastewater permit (WQ0003698) contains groundwater
monitoring requirements that are reported to our Information Processing Unit in Raleigh. The Asheville
Regional Office is requesting that a separate electronic copy (preferred) or hard copy of the monitoring
report be sent to the regional office for the foreseeable future. The monitoring report should be sent on
or before that last working day of the month following the sampling month.
The monitoring report should include the GW-59 form, GW-59A form, laboratory analytical report, and
any available field data (i.e., temperature, conductivity, dissolved oxygen, pH, oxidation reduction
potential, water level, and turbidity). The monitoring report can be sent C/O Brett Laverty to
brett.laverty@ncdenr.gov or 2090 U.S. Highway 70 Swannanoa, NC 28778.
