This is an approximately 73 acre tract located in a rural farming area that is changing to single family subdivisions. The tract is approximately 50% wooded and 50% crop land. A major
power line cuts diagonally across the middle of the property running NW to SE. The property fronts on Steven's Chapel Road.

4h. Explain the purpose of the proposed project: *
Purpose is to provide single family homesites for the growing population of this area. The developer will provide paved streets and utilities. The septic will be onsite on each building lot.
The project should provide approximately 45 building lots.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
As previously stated,the project should provide 45 building lots. The entrance to the project will be street "A"entering from Steven's Chapel Road. The only wetland, stream, buffer impact
will be a necessary stream crossing to access lots on the other side.
A bull dozier will clear the streets and a motor grader will grade the streets. The streets will be paved with asphalt. The crossing will be installed with a trackhoe. The utilities will be
provided by the developer and septic will be on -site on each individual building lot. I don't foresee any indirect impacts caused by this project. A storm water and sedimentation and erosion
control plan will be approved by the County and implemented before construction begins.

The stream has a small meander where the culvert is being placed, we counted the meander (831f) in the impact making it longer than the actual 72 ft
length. In addition to the 83 Ift impact there is 16 ft of riprap that will be placed flush with the existing stream bottom so the impact will not count toward
mitigation.

There are 3 streams existing o the tract. Care was taken in design to avoid steam impacts as much as practical. There will only be 1 stream crossing
and it is on Stream 1. The crossing is perpendicular as possible to minimize the impact to stream, wetland and buffer.

All wetlands, streams and stream buffers will be clearly marked in the field to help avoid accidental impact with machinery. Wetlands will be protected
with silt fence in areas of construction. All disturbed riparian areas will be provided with a temporary ground cover. The crossing will not commence when
substantial rainfall is forecasted and not installed during periods of high stream flows.

IPaC-list attached, 1) Red -cockaded Woodpecker -"The red -cockaded woodpecker makes its home in mature pine forests." No habitat, no affect. 2)
Neuse River Waterdog- "They are usually found in streams wider than 15 meters (m), deeper than 100 centimeters (cm), and with a main channel flow
rate greater than 10cm/second" This is an intermittent first order stream, no habitat, no affect. 3) Carolina Madtom-" Prefer clean water with permanent
flow and are not tolerant of siltation and turbidity." This is a first order intermittent stream that drains from a row crop farm field,no habitat, no affect. 4)
Atlantic Pigtoe-"The Atlantic pigtoe is dependent on clean, moderate -flowing water with high dissolved oxygen content in creek and riverine
environments." This is a first order slow moving intermittent stream that drains from a row crop field, no habitat, no affect. 5) Tar River Spinymussel-"The
Tar River spinymussel lives in relatively silt -free uncompacted gravel and/or coarse sand in fast -flowing, well oxygenated stream reaches." As stated
above, this is a slow moving , intermittent first order stream sand bed stream
with sediment from the farm field. No habitat, no affect. 6) Monarch Butterfly -"Are found in meadows, roadsides, and sandy areas where milkweed plants
grow." In the farm field or in the woods was no evidence of milkweed, have never seen any Monarch Butterflies on the project. no affect.

The North Carolina Natural Heritage Program (NCNHP) appreciates the opportunity to provide
information about natural heritage resources for the project referenced above.
Based on the project area mapped with your request, a query of the NCNHP database indicates that
there are no records for rare species, important natural communities, natural areas, and/or
conservation/managed areas within the proposed project boundary. Please note that although there
may be no documentation of natural heritage elements within the project boundary, it does not

The primary information used to generate this list is the known or expected range of each species. Additional areas of
influence (AOI) for species are also considered. An AOI includes areas outside of the species range if the species could be

Explanation: The feature(s) listed above that are checked "Subject" to the Johnston County Neuse
Riparian Buffer Ordinance (Section 14-393 Johnston County Code of Ordinances) has or have been
approximately shown on either the Soil Survey of Johnston County, North Carolina (1994) or the most
recent version of the 1:24,000 (7.5 minute) quadrangle topographic maps prepared by the United States
Geologic Survey (USGS), and have been located on the property and possess characteristics that qualify
it to be a stream. Each feature that is checked "Not Subject" has been determined not to be a stream or is
not present on the property. There may be other streams located on your property that do not show up on
the maps referenced above but still may be considered jurisdictional according to the US Army Corps of
Engineers and/or the North Carolina Division of Water Resources.
This on -site determination shall expire five (5) years from the date of this letter. Landowners or
affected parties that dispute a determination made by Johnston County may request a
determination by the Director of the Division of Water Resources c/o the 401 and Buffer Permitting
Unit, or its successor. An appeal request must be made within sixty (60) days of the date of this
letter or from the date the affected party (including downstream and/or adjacent owners) is notified
of this letter. A request for a determination by the Director must be submitted in writing with a
copy of this letter to:

This determination is final and binding unless, as detailed above, a request for an appeal has been
made in writing within sixty (60) days.

subject property (stated above). This project may require a Section 404/401 Permit for the
