Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes no major changes from the draft permit sent to you on January
30, 2013. We cannot grant your request to remove condition A. (4). The most recent site inspection noted
that solids management at your facility is inadequate for permit compliance.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local

As of this permit issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions included herein.
Knightdale Estates MHP Limited Partnership is hereby authorized to:

Basin, at the location specified on the attached map.

1. U: at least 50 feet upstream from the outfall. D: at least 200 feet downstream from the outfall.
2. If solids management at the facility is deemed inadequate, this permit may be reopened to increase TSS
monitoring to 3/Week (see A. 4).
3. The facility shall report all effluent TRC values reported by a NC certified laboratory (including field certified
laboratories). However effluent values < 50 µg/L will be treated as zero for compliance purposes.
4. For a given wastewater sample, TN = TKN + NO3-N + NOz-N, where TN is Total Nitrogen, TKN is Total Kjeldahl
.Nitrogen, and NOa-N and NO2-N are Nitrate and Nitrite Nitrogen, respectively.
5. Total Nitrogen Load is the mass quantity of Total Nitrogen discharged in a given period of time [see A. (2)
Calculation of Total Nitrogen Loads].
There shall be no discharge of floating solids or visible foam in other than trace amounts.

A.(2.) CALCULATION OF TOTAL NITROGEN LOADS
a. The Permittee shall calculate monthly and annual Total Nitrogen (TN) Loads as follows:
i. Monthly TN Load (pounds/month) = TN x TMF x 8.34
where:
TN = the average Total Nitrogen concentration (mg/L) of the composite samples
collected during the month
TMF = the Total Monthly Flow of wastewater discharged during the month
(MG/month)
8.34 = conversion factor, from (mg/L x MG) to pounds
ii. Annual TN Load (pounds/year) = Sum of the 12 Monthly TN Loads for the calendar
year
b. The Permittee shall report monthly Total Nitrogen results (mg/L and pounds/month) in the
Discharge Monitoring Report (DMR) for that month and shall report each year's annual
results (pounds/year) in the December DMR for that year.

A. (4) SOLIDS MANAGEMENT REPORTS
The Permittee shall record solids depths in the clarifier(s) (using a sludge judge), sludge wasting
events, onsite sludge storage capacities, effluent TSS concentrations, and all sludge hauling
records (frequency and volume). This information must be documented for each ORC visitation at
the treatment facility. Solids data must be provided in a written report detailing how residuals
are appropriately managed.
• Reports must be submitted to NC DENR Raleigh Regional Office, Surface Water Protection
Section.
• Reports are due by June 30st and December 31st of each calendar year.

Dear Mr. Murphrey:
The subject inspection was performed on March 19, 2013 by Sara Knies and Ted Cashion of this
office, and Eric Green with the Wake County Environmental Services. Your cooperation and that of
Guido Carrara, ORC, was appreciated. Findings during the inspection were as follows:
1. The current NPDES permit expired on February 28, 2013. You submitted a permit renewal
application that the Division received on August 27, 2012. The permit application is under
review by the Division.
2. A review of lab data and DMR data for September and October 2012 showed that on October
18, 2012 the DMR result for TSS was 8.6 mg/1 but the lab report from Pace showed TSS at 86
mg/1. Mr. Carrara contacted Pace during the inspection and later emailed a corrected report
from Pace noting the TSS lab result as 8.6 mg/1 (instead of 86 mg/1).
A review of DMR data from January 2012 - December 2012 showed all monitoring was
performed as required by the permit. A daily maximum fecal coliform violation occurred on 8-
9-12. A daily maximum TSS violation occurred on 3-22-12. These violations were previously
addressed in separate Notices of Violation.
3. The package treatment plant is permitted at 0.025 mgd and consists of the following
components: manual bar screen, aeration basin with diffused air, primary and secondary
clarifiers, tablet chlorinator, chlorine contact chamber, ISCO 3230 bubbler flow meter with a
60° V-notch weir, continuous chart recorder, post aeration, tablet dechlorination, sludge
digester, and 2 above ground concrete sludge holding tanks. The discharge is to an unname4ne

tributary to the Neuse River, Class C-NSW. There is also a pump station that serves the
mobile home park. This station was inspected in August 2012 and was not inspected during this
inspection. The collection system consists of terra cotta pipe. It is recommended that
improvements to the collection system be implemented.
4. Mr. Murphrey stated 110 trailers can be tributary to the treatment plant. Of these, currently
there are 104 trailers connected, with 6 lots vacant. There are also 6 lots that are on septic tank
systems. The total number of trailers at the park is 116. Mr. Carrara followed up with our
office in a telephone call to note that the water system at the site has an ion exchange system to
remove radiological contaminants, and that the backwash from this system is discharged to the
wastewater treatment plant. The NPDES permit may be modified to reflect this backwash
waste stream that is tributary to the wastewater treatment plant.
5. Screenings from the manual bar screen are disposed of in a plastic drum and taken to the onsite
dumpster for pickup. The water level in the aeration basin was at a level that it appeared debris
on the bar screen could be washed off and into the aeration basin. Please make adjustments to
the bar screen or water level as needed to prevent debris from entering the treatment zone.
6. The aeration basin D.O. reading during the inspection was 3.66 mg/1. Mr. Carrara stated the
D.O. reading the previous day was 1.54 mg/1. He stated there is no valve to regulate the D.O. in
the aeration basin to keep a more consistent D.O., and that the air is on constantly and does not
cycle on and off. At the time of the inspection Mr. Carrara was blowing off excess air to reduce
the D.O. in the basin. The color of the mixed liquor appeared satisfactory for good operation of
the system. The foam on the surface of the aeration basin was not excessive, covering
approximately 20% of the basin surface. There appeared to be an excessive amount of
turbulence in part of the aeration basin. This can cause the floc to shear, causing undesirable
settling characteristics in the clarifier. It is recommended you investigate installing controls
on the aeration system to prevent excessive turbulence in the aeration basin. The diffused
air headers may need to be checked also to ensure the system is intact.
Mr. Carrara stated that in the past bristle worms have been present in this plant. When this
occurred, he stated these worms ate sludge and he was unable to waste any sludge from the plant.
Due to the nature and volume of flows experienced at the plant, you may wish to investigate
whether the addition of an aerated equalization basin to stabilize flows through the plant may
improve the operation of the plant.
7. A '/a pound bag of "Biobug F.P." is added in the aeration basin daily to control grease.
"SELFLOC 1832" (Southeastern Labs) is dripped in at the head of the aeration basin to promote
floc formation at a rate of one pound/2 days.
8. Mr. Carrara conducted a 30 minute settleometer test during the inspection. The 30 minute result
was 350 ml, with some suspended floc in the supernatant, and grease floating on the surface.
9. The primary and secondary clarifiers had some grease on the surface, with more grease on the
primary clarifier than the secondary clarifier. Two surface skimmers removed the grease from
the clarifiers and appeared to be operating satisfactorily. A sludge judge measurement showed
the sludge blanket to be 7 feet deep. The clarifier depth is 11 feet. Sludge blankets should be
no more than 1/3 the depth of the clarifier. Sludge blankets greater than 1/3 the depth of the
clarifier have an increased risk of solids being washed out and discharged to the receiving
stream. Pin floc was observed being discharged from the clarifier.

Please ensure residents in the mobile home park are aware that grease is not to be disposed of
down the drain.
10. There were 4 tubes of chlorine tablets prior to the chlorine contact chamber. A review of the
label showed the tablets were approved for wastewater disinfection.
11. A sludge judge measurement was performed in the chlorine contact chamber. There was a one
foot sludge blanket in the 3 foot depth contact chamber. Mr. Carrera stated the solids in the
contact chamber are pumped to the digester weekly. This amount of sludge accumulation in a 1
week period is an indicator that proper settling is not occurring in the clarifier, and/or a high
volume of solids are retained in the clarifier and are being carried over from the clarifier to the
contact chamber. As noted in item # 10, pin floc was observed being discharged from the
clarifier. Solids accumulated in the chlorine contact chamber have an increased risk of being
washed out and discharged to the receiving stream.
12. The ISCO 3230 bubbler flow meter was calibrated 10-2-12 by Atlantic Environmental. You are
reminded the flow meter must be calibrated at a minimum of once per year (365 days) and
maintained to ensure the accuracy is within 10% of the true flow rate.
13. The tablet dechlorinator was supplied with appropriate tablets.
14. The effluent appeared clear at the time of the inspection. A grab sample of the effluent was
taken during the inspection and analyzed for TSS. The lab result was 6.2 mg/1, which is within
permit limits.
15. Mr. Carrara stated that effluent grab samples are collected from the end of the discharge pipe on
Thursdays and that he transports the samples to Pace Labs.
16. The facility has a 2500 gallon aerobic digester, and two-1000 gallon above ground concrete
sludge holding tanks, for a total capacity of 4500 gallons. At the time of the inspection the
aerobic digester was approximately 'A full of sludge. The 2 sludge holding tanks were being
decanted to thicken the sludge. One of the 2 sludge holding tanks was full of sludge and was in
the process of being thickened. The other sludge holding tank had been thickened and was
approximately %2 full of sludge.
Mr. Carrara stated that Granville Farms hauls sludge from the plant every 2 weeks. He
explained that the practice is to lime stabilize the sludge in the digester, (where he documents
the pH and time), then to pump the lime stabilized sludge to the two above ground concrete
sludge holding tanks. However, as discussed during the inspection, the two sludge holding
tanks already contain a volume of sludge that has been wasted from the plant and has not been
lime stabilized. Adding the lime stabilized sludge from the digester to the unstabilized sludge
contained in the 2 sludge holding tanks does not constitute compliance with the 40 CFR
regulations for the sludge in the 2 holding tanks. The pH in the sludge holding tanks is not
documented. Therefore, the total volume of sludge in the 2 holding tanks has not been properly
lime stabilized, and is in violation of the 40 CFR 503 Standards for the Use and Disposal of
Sewage Sludge. Mr. Carrara stated he will modify his procedure to ensure all sludge has been
lime stabilized properly, and will begin to document the pH and holding time of the sludge in
each of the 2 sludge holding tanks to comply with the 40 CFR 503 requirements.

You may wish to consider adding a diffused air system in the sludge holding tanks to assist with
mixing the sludge and lime during stabilization of the sludge.
It is also recommended that additional sludge holding capacity be incorporated at the facility to
increase the current 2 week holding capacity, and to assist the facility in complying with the
vector attraction and pathogen reduction requirements noted in 40 CFR 503.
This office is aware that Poplar Creek subdivision, tributary to the City of Raleigh sewer system, is
being constructed off Poole Road in close proximity to your facility. Part III Section D of your NPDES
permit requires you to evaluate all wastewater disposal alternatives and pursue the most environmentally
sound alternative of the reasonably cost effective alternatives. Please investigate connection to the
City of Raleigh sewer system and inform this office of your intention to connect to the City of
Raleigh sewer system.
Please submit the following records and information to our office for review no later than May 8,
2013:
1. Sludge hauling and lime stabilization records for October 2012 — March 2013.
2. A copy of the chart recorder flow readings for October 2012 — March 2013.
3. A copy of the operator records, including sludge judge measurements, for October 2012 — March
2013.
4. Verify that lime stabilization of sludge is now being properly performed in the two sludge
holding tanks, and that pH and holding times are documented in all sludge holding tanks and the
digester, to ensure compliance with the 40 CFR 503 regulations.
5. Verify that operational procedures will be implemented to reduce the sludge blanket depth in the
clarifier to no more than 1/3 the depth of the clarifier.
6. Please inform this office of your intention to increase sludge holding capacity at the facility.
Please contact this office for any permitting modification/Authorization to Construct approvals
that may be needed with these plant additions.
7. Please inform this office of your intention to connect to the City of Raleigh sewer system.
Given the historical sludge management concerns at the facility and potential impacts to the receiving
stream, this office will careful review your written response as well as your efforts/intention to connect
to the City of Raleigh sewer system. Further, this office is also considering conducting a formal
stream impact assessment (in concert with DWQ's Environmental Science Section) to further
identify water quality impacts and the identification of stream standard violations that may be
coincident to the discharge and/or historic sludge management practices.
You are reminded to contact this office of any situation that potentially threatens public health or the
environment.

Good morning Charles,
I checked the draft permit to see exactly what condition A(4) stated and pasted it below. This
information should be collected and retained by any ORC at any facility for proper operation, not
just at this facility.
The last CEI at the facility (Sept, 17, 2012), noted there were 7 feet of solids in the 11 foot deep
clarifier. The sludge blanket should be no more than 1/3 of the clarifier depth. Solids can escape
easily with too high of a blanket. There was also a high amount of suspended solids in the
clarifier. There was also a buildup of sludge in the chlorine contact chamber. NOV-2012-PC-
0300 was issued in part due to these findings.
These findings indicate poor operation of the facility at the time of the inspection. We
recommend the A(4) condition remain in the permit.

This letter is in response to your letter of January 30 enclosing the above draft.
After seeking advice from our state certified and licensed operator, we hereby
reject the A. (4) Solids Management reporting condition that the Regional Office wants to
add.
Our plant is compliant and water going downstream out of the facility is cleaner than the
water upstream even though the collection system is not a state of the art, new plant. But
we feel this plant would be non -compliant with this A. (4) condition, resulting in major
penalty dollars having to be spent. Several years ago we had to pay thousands of dollars
because of operator errors. Now we have a very competent one and he says adding this
condition would be a huge mistake.
Thank you for your assistance in removing the A. (4) from this proposed permit.

The RRO recommends the permit be reissued. The Permittee has installed 2 -1200 gallon septic tanks for sludge in addition
to the plants digester. Tablet dechlorination has been added since the last permit issuance. Post aeration is provided at the
end of the contact chamber. The RRO has no record of A to C's being issued for the 2 septic tanks and the post aeration
units. No primary clarifier is noted in the inspection report (it is noted in the permit application but it appears by the
Permittee's description it is after the aeration basin, making it a secondary clarifier). Please adjust the treatment unit
description in the permit to reflect the current units. The receiving stream is very small. It appears the limits are very relaxed
for this stream. Please confirm the 7Q10 and 30Q2 for this UT to the Neuse River and adjust limits as needed.
Due to solids depths observed during inspection the RRO request the following in the permit:
Please consider increasing the TSS monitoring (e.g. 3 times per week or greater) for the first two years of the
permit cycle.
Solids management plan and reporting. Record clarifier(s) solids depths (using a sludge judge), sludge wasting
records, onsite sludge storage capacities, effluent TSS concentrations, and all sludge hauling records (frequency
and volumes). This information must be documented for each ORC visitation at the treatment facility. A written
report must be provided every 6 months. This data must be provide and summarized in a written report detailing
how residuals are appropriately managed. (reports must be submitted to Raleigh Regional Office — Surface Water
Protection Section and are due by June 30st and December 31St) )

We discovered this week that several permits for small Aqua NC facilities were given TN limits during their 2008
renewals (should have 'Monitor & Report' only for TN). This prompted Ron to review all permits for Neuse
dischargers < 0.5 MGD. Besides finding one other facility that got a limit by mistake, he found these three that did
not have the new special condition listing their TN allocations. Permit writers should add the special condition at the
next renewal or mod.

The Knightdale Estates Wastewater treatment system is comprised of a 25,000 gallon per
day conventional aerobic digestion plant equipped with an aerobic digester of 2500
gallons. In addition, there are two(2) 1200 gallon anaerobic digesters for additional
storage. All domestic waste from the mobile home park enters the plants aeration basin
and is gravity fed to a primary and secondary rectangular clarifier system. Discharge
from the final clarifier is directed to a chlorine tablet feeder, chlorine contact chamber
(equipped with an air diffuser) and through a dechlorination tablet feeder system.
Wastewater flow is monitored continuously utilizing an ISCO 3230 bubble flow meter.
Design removal values for BOD, TSS, Nitrogen and Phosphorous are expected to be 75-
95%, 85-90%, 30-70% and 10-30% respectively.
