TO WHOM IT MAY CONCERN:
IN REGARDS TO NOTICE OF VIOLATION
NOV-2022-LV-0320
On January 28, 2022, we asked for assistance with the Bethany Wastewater Plant (NC0037001)
and received a visit from Jordan with NCDENR from the Winston Salem Branch. His suggestion
to us was to begin recording PH on the head tank daily to establish baseline data. He stated we
could add lime or soda ash to increase and stabilize the PH. We already collect data on the PH
from the recirculation tank. Jordan asked us to record the head tank PH daily and see if we
could see any wild fluctuation there.
On approximately April 15, 2022, we installed an extra 2 inch electric open/close valve on the
discharge line. We have only discharged 100 gallons/per day for sampling. As of May 17, 2022,
we are repiping the sample point to the recirculation line. We only discharge when samples are
correct. Please let me know if you need any additional information. Please explain the process
for Condition 6 in Section E (email, phone, etc ) Thank you for your time and consideration in
this matter.
