The don't need to be updated but 001 should indicate Pretty Creek in the description. I've also attached to letter of ROS
for outfall 004. The ROS form for 003 and 006 have not been sent yet and we will follow up once that has been done.

Also, do the latitude and longitude values need updating? You have them listed in decimal degrees, but we have to enter
them as degrees, minutes, seconds, and so they may differ a bit when rounded than what you have below. Before
making any revisions, please let me know if these are already accurate:

Have you submitted the info about the ROS certification? I just want to verify the ROS status before I enter these. Or, if
this was a recent approval, just send me the ROS approval documentation.

If you had Representative Outfall Status in the previous permit term, attach the completed ROS
Renewal Certification Form and prior documentation that was provided when the Division originally
granted ROS to the Stormwater Upload Form.
