Tata GOVERNOR SECRETARY April 28, 2014 U.S. Army Corps of Engineers Regulatory Field Office 2407 West 5 "' Street Washington, N.C. 27889 Attention: Tracey Wheeler Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of Bridge 425 over Unnamed Tributary to Perquimans River on SR 1001 (Turnpike Road) in Perquimans County. WBS # 17BP.1.R.46 The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit and corresponding DWQ certification for the replacement of Low Impact Bridge #25 in Perquimans County. The bridge is located on SR 1001 approximately 7.0 miles southeast of the intersection of NC Highway 158 and spans an Unnamed Tributary to the Perquimans River. The receiving stream fed by the unnamed tributary is classified by DWQ as class C;Sw waters. Bridge #25 is currently a single span pre- stressed concrete channel deck on concrete caps and timber piles; DAL = 30'. This structure will be removed using NCDOT's guidelines for bridge removal. The proposed bridge will be placed on its existing alignment. The new bridge structure will be a single -span 18" cored slab type bridge; OAL = 45' long by 30' wide. The proposed bridge will have vertical sheet pile abutments at the end bents. There are no jurisdictional wetlands present on site. Structural sheet pile and associated fill from the vertical sheet pile retaining wall results in 44 linear feet of permanent stream impacts. An off -site detour will be utilized during construction. This bridge is following the Low Impact Bridge Process established by FHWA, NCDOT, and the Regulatory Agencies. Included with this letter are the location map, permit drawings, and Low impact Data Sheet. If you have questions please call me at (252) 482 -1876.
