Subject: Permit Modification Request for NC 58 Safety Improvements (DWQ
20131039) over a UT to Toisnot Swamp in Wilson County (WBS 45423.1.1)

Dear Mr. Ridings,

The purpose of this correspondence is to request a modification of the 401 permit issued
for impacts associated with safety improvements to NC 58 north of SR 1626 (Fairfield
Dairy Rd.) in Wilson County. Specifically, we request permission to hand clear an
additional .16 acre of wetlands on the west side of NC 58 to accommodate utility
relocation. An additional 2,121 square feet of buffer impacts will also be incurred as a
result of these activities.

The additional work will be done within a utility easement. Please review the revised
permit drawing that accompanies this correspondence for the exact location and
dimensions of the area to be impacted by the requested permit modification. Please
contact myself or Chad Coggins with any questions, comments, or concerns you may
have regarding this request.
