This letter is in response to your request dated April 28, 2014, to authorize a change in impacts for a culvert headwall
installation at Site 1 on the R -2583 project.
Your request indicates that during construction of the project, it was determined that the headwall unit for the culvert
permitted at Site 1 will permanently fill approximately 48 sq. ft. of Section 404 Wetlands, approximately 16 sq. ft. of
surface waters, and approximately 4 linear feet of jurisdictional stream that were not accounted for in the CAMA Major
Permit. The 48 sq. ft. of Section 404 Wetland area to be impacted was previously permitted for mechanized clearing.
This Letter of Refinement authorizes the change in impacts as described in your request letter dated April 28, 2014,
including the attached workplan drawings (2) dated as received on May 7, 2014. This letter shall be attached to the
original of CAMA. Permit No. 63 -12 which was issued on June 7, 2012, and both documents shall be readily available on
site when a DCM representative inspects the project for compliance. All conditions and stipulations of the active permit
remain in force unless altered herein.
