This letter is in response to your request, dated April 28, 2014, to authorize a change in impacts for a culvert headwall
installation at Site 35 on the R -2507A project.
Your request indicates that during construction of the project, it was determined that the headwall unit for the culvert
permitted at Site 35 will permanently fill approximately 22 sq. ft. of Section 404 Wetlands and will require approximately
234 sq. ft. of temporary hand clearing of Section 404 Wetlands that were not accounted for in the CAMA Major Permit
issued for this project.
This Letter of Refinement authorizes the change in impacts as described in your request letter dated April 28, 2014,
including the attached workplan drawing dated April 22, 2014. This letter shall be attached to the original of CAMA
Permit No. 92 -12, which was issued on August 3, 2012, as well as all subsequent modifications and refinements, and all
documents shall be readily available on site when a DCM representative inspects the project for compliance. All
conditions and stipulations of the active permit remain in force unless altered herein.
