All,
I was looking over the additional information you provided on September 8, 2021. Everything looks good except for a
few small issues:
Please correct the following:
- Original Comment 9 (Prior Comment 2) – Please provide an original signature (i.e., in ink) on the O&M
Agreement form as required by Section VI, 3 of the Application and 15A NCAC 02H .1042(2)(j). Please provide a
hard copy of this form with an original signature. The provided hard copy appears to be a photocopy.
- Original Comment 13 (Prior Comment 4) – Please include the wetland determination in the main set of plans as
required per Section VI, 8m of the Application and 15A NCAC 02H .1042(2)(g)(iii). For example, if it was
determined that there are no wetlands located on-site a note must be placed on one of the plan sheets
indicating that there are no wetlands located on-site and listing the name/qualification of the individual that
made this determination (the note could read something like, “There are no wetlands located within the project
area. Determination made by John Smith of ExampleCorp”, or something to this extent).
- Prior Comment 7 – Please provide a second hard copy of the revised plan sheets provided on 9/8/21 as required
per Section VI, 8 of the Application and 15A NCAC 02H .1042(2)(g). We require two hard copies of the plan
sheets since we will hold onto a copy of the approved plans and will provide a copy of the approved plans to the
applicant for them to keep on record.
Please provide the following:
- The original signed O&M Agreement Form (originally signed on 8/31/21). If it is not possible to get the original
signed form, you will need to provide a new original signed O&M Agreement Form (and electronically upload a
copy of this new version).
- 2x hard copies & 1x electronic copy of the revised plan sheet that includes the wetland determination.
- 1x hard copy of the applicable plan sheets provided in the 9/8/21 submission (so that I have a total of 2x hard
copies of the plan sheets submitted on 9/8/21).
NOTE: We are currently experiencing some issues with Laserfiche (the electronic file upload form). If you plan on
resubmitting this week, please email me the electronic files and I will manually add them to the electronic permit file
once the issues are resolved.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
permit without doing another formal 30-45 day review.
