Processing Asst., NCDO'I' Greenville Const., 252- 830 -3495 Mr. Kincannon called the meeting to order and the following items were reviewed with discussion as noted: SCRIPTION: This contract is for the replacement of' Bridge 69 in Pitt County and Bridge 34 in Greene County. Bridge 469 will be replaced by a cored slab ridge with approach slabs with a superstructure consisting of 2 spans with steel pile foundation. Bridge 434 will be replaced by a cored slab bridge with approach slabs with a superstructure consisting of 3 spans with steel pile foundations. Only the control points with a reference station and benchmark location will be furnished on an initial one time basis. All other engineering. surveying, layout and measurements shall be responsibility of the contractor. Thompson and Associates will provide surveying services for the Contractor. Mr. Kincannon suggested the surveyor needs to take ground shots prior of work beginning and compare to data that has been provided in order to avoid any last minute grade changes. CONTRACT TIME AND LIOUIDATED DAMAGES: The date of availability for this contract is not before April 14, 2014 or later than May 12, 2014. The completion date for this contract is 90 consecutive calendar days after completion of Intermediate Contract Time #1. The liquidated damages for this contract are Two Hundred Dollars ($200.()()) per calendar day. INTERME.DIATE•, CONTRACT TIME #1: The Contractor will be required to complete all work included in this contract and shall place and maintain traffic on same. The date of availability for the intermediate contract time is the date of availability of the contract. The completion date of this intermediate contract time is the date which is One I fundred and Seventy -five (175) consecutive calendar days after the date of availability. The liquidated damages for this intermediate contract time are Seven Hundred Fifty Dollars (S750.00) per calendar day. The Contractor anticipates starting work on April 28, 2014. Jim Evans with Traffic Services stated this would not allow enough time to locate the signs, etc. and advised that if Traffic Services started work today they could have it ready by the April 29`h. Mr. Kincannon noted that the contract does state a two (2) week notice is required and the one day delay from the Contractor's original date of the 28`1' would be the best the Department could offer. Mr. Evans also asked what time they plan to start and the Contractor stated they would like to start by 8:30 am. Mr. Evans advised Traffic Services would get the detour signs in place and requested the road closure by put in TIMS to advise emergency personnel and local municipalitics of the closure. PERMANENT VEGETATION ESTABLISHMEN T: All work for initial vegetation planting shall be performed as a part of the work necessary for the completion and acceptance of the Intermediate Contract Time. Between the time of ICT and Final Project acceptance, or otherwise referred to as the vegetation establishment period. the Department will be responsible for preparing the required National Pollutant Discharge Elimination System (NPDES) inspection records. Once it has been deterniined that the permanent vegetation establishment requirement has been achieved at an 80% vegetation density the Contractor will be notified by Roadside Environmental (Andy Blankenship or Carla Pugh) to remove the remaining erosion control devices that are no longer needed. MAJOR CONTRACT ITEMS: The only major contract item for this contract is Line Item 59 — Prestressed Concrete Cored Slabs, SPECIALTY ITEMS: Specialty items for this contract consist of guardrail. erosion control. and reforestation. FUEL PRICE ADJUSTMENT: The base price index for Diesel #2 Fuel is $3.1476 per gallon which applies to asphalt line items as listed on Page 7 of the Contract. DBE GOALS: The DBE goal was met for this project. Mr. Kincannon reminded the Contractor that in the event they need to replace a DBE to be sure to follow the guidelines as outlined in the contract beginning on Page 17. All correspondence regarding DBE. replacement must go through the Resident Engineer's office. DOMESTIC STEEL: All steel and iron products that are permanently incorporated into this project shall be produced in the United States. Domestically produced high strength fasteners are required. 12 MONTH GUARANT The Contractor shall guarantee materials and workmanship against latent and patent defects arising from faulty materials, faulty workmanship or negligence for a period of twelve months following the date of final acceptance of the work for maintenance and shall replace such defective materials and workmanship without cost to the Department. This guarantee provision shall be invoked only for major components of work in Nvhich the Contractor would be wholly responsible for under the terms of the contract. Examples would include pavement structures. bridges, and sign structures. This provision will not be used as a mechanism to force the Contractor to make repairs or perform additional work that the Department would normally compensate the Contractor for. Routine maintenance activities such as mowing grass, debris removal, etc. are not part of this guarantee. SUBMITTALS: The Contractor provided the following submittals on the day of the meeting: Project Personnel. Authorized Personnel to Sign Supplemental Agreements, EEO Officer, Date of Estimate Request. Source of Materials, Proposed Subcontractors, Level 11 Erosion Control Personnel, and Projcct Schedule (with exception of narrative). Mr. 4 Kincannon advised that Subcontractor Approval Forms must be submitted prior to any work being performed by the subs. EROSION CONTROL: Reference was made to page 27 of the contract regarding the Contractor's responsibility for operations and subcontractors' operations to ensure that the Erosion and Sediment Control /Stormwater Pollution Prevention Plan is implemented and maintained over the life of the contract. Mr. Kincannon reminded the Contractor there must be a Level 11 person on site at all times when work is being performed on the project. The Contractor stated that Joe Knoll will be the Level I1 person on this project. M & J Seeding will be the installer. CLEARING AND GRUBBIN Clearing and grubbing will be by Method lI as shown on Standard Drawing No. 200.02 of the 2012 Roadway Standard Drawings. PRICE ADJUSTMENT — ASPHALT BINDER FOR PLANT MIX: The base price index for asphalt binder for plant mix is $559.29 per ton. Final surface testing is not required. S. T. Vv`ooten will be performing the paving operation for this project. It was noted that since the two bridges are in close proximity to one another, the all the paving will be done at one time. PREFORMED SCOUR HOLE WITI I LEVEL SPREADER APRON: Reference was made to Page 40 of the contract regarding required submittal from the manufacturer showing the chemical and physical properties of the mat and conformance ofthe mat with the specification. Also. the contract states the surface of the soil shall be smooth, firm, stable and free of rocks, clods, roots or other obstructions that would prevent the mat from lying in direct contact with the soil surface. Page 41 shows the preformed scour hole if paid per each per line item. SIIOULDLR AND SLOPE BORROW: Mr. Kincannon asked if the Contractor plans to use a commercial pit and Mr. Knoll was advised if they do. they will need to provide information on the pit to the Resident "s office. Reference was made to Page 51 of the contract regarding specifications for the soil that can be used. SAFETY FENCE: Safety fence must be installed prior to any land disturbing activities. The fencing is to be installed along the outside riparian buffer, wetland, or water boundary, or other boundaries located within the construction corridor to mark the areas that have been approved to infringe within the buffer. wetland, endangered vegetation, culturally sensitive areas or water. Mr. Kincannon advised the Contractor to have the area staked out in order for the Department survey party to verify prior to installing the fencing. Mr. Blankenship advised the Contractor to be sure the permit drawing is used to stake out the area and not the erosion control drawing. ENVIRONMENTALLY SENSITIVE AREAS: This project is located in an Environmentally Sensitive Area. The ESA is defined as a 50 -foot buffer zone on both sides of the stream or depression measured from top of streambank of center of depression. In areas identified as ESA. the Contractor may perform clearing operations but not grubbing operations until immediately prior to beginning grading operations as described in Article 200 -1 of the Standard Specifications. Erosion control devices shall be installed immediately following the clearing operations. Once grading operations begin in identified ESA. work shall progress in a continuous manner until complete. All construction in these areas shall progress in a continuous manner such that each phase is complete and areas are permanently stabilized prior to beginning of the next phase. CRIMPING STRAW MULCH: Crimping shall be required on this project adjacent to any section of roadway where traffic is to be maintained or allowed during construction. In areas within six feet of the edge of the pavement straw is to be applied and then crimped. Mr. Kincannon stated matting would not be allowed as a substitution for straw, tack and crimping. STABILIZATION REQUIREMENTS: Reference was made to Page 50 of the contract regarding the stabilization requirements. Temporary or pennanent ground cover stabilization shall occur within 7 calendar days from the last land- disturbing activity with the following exceptions in which temporary or permanent ground cover shall be provided in 14 calendar days from the last land - disturbing activity: Slopes between 2:1 and 3:1, with a slope length of' 10 ft. or less Slopes 3:1 or flatter, with a slope of length of 50 ft. or less Slopes 4:1 or flatter 6 Andy Blankenship reminded the Contractor to be sure the NPDES forms are updated on a weekly basis and on site at all times. He also indicated that Carla Pugh may be the contact person for this project. Andy advised there is reforestation in the contract_ however. this is only in the event we disturb outside of what will be maintained in the future. I Ion stated that the permit drawings show a Type A check dam inside the buffer zone and typically we do not put anything inside the buffer zone. Andy asked if the ditch goes through the buffer zone and Stanley indicated it does. Andy stated if it goes all the way to the stream is okay to put the "Type A check dam inside the buffer. Andy said it must have been determined that a wattle would not be sufficient and therefore the check dam was required. Stanley advised he had been by the project and there is a large volume of water going through this ditch and a wattle would most likely not be enough to handle it. PERMIT: Mr. Ward discussed the following conditions of the 401 Water Quality Permit for Bridge 469 but stated the conditions are the same for both bridges: Condition 2, 3, 4, 5. 6, 7, 10, 1]. 12. 17, 18. 19.20. 22, 23, 25, 26. 27 and 28. Mr. Ward also discussed some of the conditions of the Water Quality 3883 such as Conditions 3 (All sediment and erosion control devices shall be removed and the natural grade restored within two (2) months of the date that the Division of' Land Resources or local delegated program has released the specific area within the project), and Condition 4. OTHER ITEMS DISCUSSED: Mr. Kincannon asked if the overhead cable line on the east side of the project was going to be an issue. `Ihe Contractor stated they would take a look at this and determine if it would need to be relocated. Reference was made to Page 73 of the contract regarding submittal of working drawings and contact persons. The Contractor will need submit their crane safety information to include who their competent person and riggers will be along with documentation of their qualifications and experience. Milton Rudd stated the test bars for steel arc to be 30 inches long per shipment and if it is epoxy coated the appropriate forms should be included. Mr. Rudd asked the Contractor who would be providing their concrete and it was stated they would self - perform from the Winterville plant. The Contractor stated they would be self-performing for the barrier rail. The guardrail will be installed by Bullington, Mr. Rudd stated paperwork on the bolts for the guardrail need to be certified as Level III. Also, the Contractor was advised they must use a pre - approved, pre - packaged grout mix and testing will be done on this mix. Mr. Kincannon stated that on Bridge 434 any existing substructures need to be removed during the bridge demolition. h .Iimmv Cox advised the Contractor that when their surveyor goes out to check grades they also need to check the existing asphalt.
