Hi, Debbie,
Please see the attached permit summary report for confirmation of the facility address revision. We do have additional
administrative help for our program now, but we are still not generating new COCs for Name/Ownership or other
address changes. With all the eDMR registrations and on -boarding we are trying to get caught up on, we don't have the
administrative resources to create letters and COCs for the volume of changes we receive. However, this report should
serve as documentation that it has been corrected (the date is noted at the top), and when the General Permit is
renewed, the changes will be reflected on that renewal Certificate of Coverage.

Our address has recently changed for our Main/East Campus (COC NCG030240). Do I need to submit another
Name/ownership change form? The new address is 1000 John Deere Road, Kernersville, NC.
Also, do you need any more information for the West Campus (NCG030646)?
I know you guys are short on help but I am curious when you think we will receive the COC's for both Campuses with the
name change information?

Since the company will change name, we need you to fill out the Name/ownership change form here. Check the box
that says Name Change. There shouldn't be any documentation upload necessary, only the final sign off.
That form is actually going to be replaced in the near future (one reason is the "save draft" feature is not working well
right now), so if you have issues, please let me know. Please fill it out for both permits.
