'ti BS No. 5C.093007 The North Carolina Department of Transportation (NCD(: T) proposes to construct SR 1240 (Blue Mud Swarnp Road) in Warren County. This roadway improvement proJect will include grade, drain, base, pave, and erosion control for the project length of 0.7 rnfle. This project ject will involve stream impacts associated with resetting pipe end Joints at three sites 'Using temporary dewatering. Sorne rip rap within the flow lines at Sites I and 2 may be necessary for stabilizing these perched pipes, Rip rap, annoring fior bank stabilization as well as for stab i I ization of a huge scour hole at the outlet of a storin, water pipe is also proposed. This project is within the Roanoke River Basin so buffer rules do not apply. Application is hereby made for NCDENR-DWR Section 401 Water Quality Certification (WQQ for Road Improvements, A pre-construction notitication, construction plan, project rnapping, and photographs are attached with this request, By copy of this letter we are notifying the USACE of the intent to work under a non- rel:iorting .~J ''P 14, If you have arty questions or need additional intbrination, please contact Mrs. Heather Montague at (252)492-0111 or hwniontqgugcrlccaclot.perw, Sincerely, V Division Engineer cc: Mr. Eric Alsmeyer, NCDOTCoordiriator, US.
