3. Project Description:
3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
This is a timber tract. We are trying to restore proper natural timber regeneration on
the tract. We have had issues with the beaver population former bad logging
practices and poor road design that impeded natural water flow on property. We have
been very successful in restoring natural water flow and have seen a great
improvement in the health of the forest in the areas we can easily access.

4. Proposed Activity
Provide a detailed description of the proposed activity including its purpose and include the type of equipment to be used:*
We would like to restore the location formerly used as a ramp and pier on our
property known as the Avon tract. We would like to remove the pier and take a 12 x
12 preformed piece of concrete and set it on the creek bank on top of the old ramp
area. I think there will be very minimal disturbance this way. There is just a small
amount of brush that would need to be removed to make a back -in path to ramp from
our existing road. We would bring in some dirt from another location instead of
disturbing roots and trees to fix low spots in path to ramp. Probably top path with
surge stone to prevent erosion. We will also remover the old debris from a cabin that
has been destroyed for many years but littering the creek bank. We would be
certainly open to any suggestions you all have in the method of accomplishing this
project. We only need this ramp to help us access this property better. We have lots
of former logging debris and beaver damage that needs to be managed so that we
can retore the natural flow of water on this property and facilitate regeneration of
timber.

D. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project.
The location was already formerly used as a boat launch and historically. The path
length is along the former path used and is the minimal length 90 feet to access area.
We will put surge stone on road to prevent erosion and sedimentation. We are going
to bring dirt form another location to fill holes in path instead of disturbing existing
root matt. We are going use a preformed concrete pad for ramp so no construction
has to take place in buffer. We will just set concrete in place.

E. Diffuse Flow Plan
la. What type of SCM are you providing?*
Level Spreader
Vegetated Conveyance (lower SHWT)
Wetland Swale (higher SHWT)
Proposed project will not create concentrated stormwater flow through the buffer
